Late Onset Ipilimumab-Induced Pericarditis and Pericardial Effusion: A Rare but Life Threatening Complication

Abstract: Metastatic cutaneous melanoma has poor prognosis with 2-year survival rate of 10–20%. Melanoma cells express various antigens including gp100, melanoma antigen recognized by T cells 1 (MART-1), and tyrosinase, which can induce immune-mediated anticancer response via T cell activation. Cytotoxic T-lymphocyte associated antigen-4 (CTLA-4) is an immune check point molecule that negatively regulates T cell activation and proliferation. Accordingly, recent phase III clinical trials demonstrated significant survival… Show more

“…Several questions remain to be answered. Immune checkpoint inhibitors are associated with significant risk of immune-related adverse events in a range of 10-40% [22,24,43,66,67], and our meta-analysis also demonstrated statistically higher grade 3/4 immune-mediated adverse events rate in the experimental group (Fig. S2).…”

“…However, from the presented data it is not clear whether this was related to the drug or to the malignant disease [9]. Recently, ipilimumab-induced pericarditis and pericardial effusion have been reported in a patient with metastatic melanoma [10]. To our knowledge, this is the first report of nivolumab-induced pericardial tamponade.…”

“…Cardiovascular abnormalities may be encountered, the most common of which is pericarditis, which can produce a pericardial effusion and/or pericardial thickening on CT. Imaging findings suggestive of ventricular interdependence and constrictive, effusive physiology (including septal bounce and respiratory septal shift) may be present on echocardiography and MR imaging . The most common complication involving the thyroid gland is thyroiditis, which may be identified on iodine‐123 thyroid scintigraphy and/or ultrasound.…”
