Late-Onset Adrenal Hyperplasia in North Indian Hirsute Women

Abstract: The occurrence of late-onset congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency was studied in 60 consecutive hirsute women by means of adrenocorticotrophin (ACTH)-stimulated serum 17-hydroxyprogesterone (17-OHP) levels. Five (8.3%) women had an exaggerated response (ACTH-stimulated 17-OHP 3,160 ± 560 ng/dl). All of them had regular periods and 3 were virilised. The other 2 were indistinguishable from those with idiopathic hirsutism or polycystic ovarian disease.

“…PKOS, üreme çağındaki kadınları etkileyen en sık endokrin anomalidir [19]. Mithal ve arkadaşlarının yapmış olduğu bir çalışmada hirsutizmi olan 60 hastanın %75'inde PKOS saptamışlardır [20]. Zargar ve arkadaşları tarafından Hindistanlı 150 hasta üzerinde yapılmış olan bir başka çalışmada ise PKOS insidansı %37,3 olarak saptanmıştır [21].…”

“…16 The disorder affects between 1 and 10% of hyperandrogenic women, depending on the ethnicity of the population studied. 2,14,[17][18][19][20][21][22][23][24][25] In the United States, we and others have observed an NCAH prevalence of 1 to 2% among populations of white and Hispanic hyperandrogenic patients. 2,14,17,18 In contrast, studies in France, Italy, and Canada yield frequencies of 4 to 6%, [19][20][21] and other studies originating in Israel, India, and Jordan demonstrate prevalences of 6 to 10%.…”

[22][23][24][25] We can conclude that there are important differences in the prevalence of PCOS and NCAH, as the frequency of PCOS is approximately 40 to 50 times higher than that of NCAH in the general population of reproductive-aged women or among hyperandrogenic patients.…”
