Laser Photocoagulation for Macular Soft Drusen

Abstract: Although no definitive conclusions should be made because of the small number of patients studied, results seem to show that this treatment does not reduce the risk of choroidal neovascularization in the treated eye of patients with a history of exudative disease in the fellow eye. It may be effective in patients with high-risk bilateral soft drusen, that is, in less advanced stages of the disease.

“…Further studies demonstrated that the drusen did not have to be treated directly with laser for resolution to occur. [38][39][40] The observation that drusen resolved to some extent after laser was applied to adjacent areas in the macula led to the development of several large, multicentered trials to assess this intervention.…”

“…These included improving the flow across Bruch membrane; reducing the lipid content of Bruch membrane and, thus, reducing an inflammatory stimulus to neovascularization; decreasing the thickness of Bruch membrane and improving its barrier ability; and stimulating RPE cells to release vasoinhibitory growth factors. 38,40,[44][45][46][47] At the same time, prophylactic thermal laser treatment to eyes with drusen is not without risk. The potential for iatrogenically induced CNV is a major concern.…”

“…[1][2][3][4][5][6][7] A hallmark feature is the presence of drusen, and increases in drusen load have been correlated with advanced stages of AMD. [1][2][3][4] Several studies have attempted drusen reduction as a means of preventing visual loss.…”

[5][6][7][8][9][10] Manual drusen quantification is laborious and costly. 11 12 Automated drusen quantification has value in furthering our understanding of the natural history of AMD and in trials of drusen reduction.…”
