Aim: The aim of this study is to compare the efficacy of LigaSure and LH procedures in the treatment of grade 2-3 hemorrhoids. Material and methods: Demographic and clinical data of the patients were recorded retrospectively. A visual analogue scale (VAS) was used to evaluate the pain intensity of the patients after the procedure and a Likert-type scale was used to evaluate patient satisfaction surveys 6 months after the procedure. Results: Of the 66 patients, (mean age: 42.12±11.92 years, %72,7 male) 34 underwent LH procedure whereas 32 underwent LigaSure procedure. Spinal anesthesia was applied for 64 patients and general anesthesia was applied for 2 patients. Procedure time and time to return to work were significantly shorter in the LH group compared to the LigaSure group (900 vs. 1200 seconds, p<0.001, and 3.64±1.29 vs. 14.46±3.73 days, p<0.001). Late complications (abscess, relapse, pruritus, and seromucous discharge) were more common in the LH group, but the difference was not statistically significant (23.5% vs. 6.3%, p=0.08). VAS pain scores were significantly lower in the LH group (p<0.001). Although there was no significant difference between the treatment groups in terms of patient satisfaction, relapse was significantly less common in the LigaSure group (p=0.045). Conclusion: In patients with grade 2-3 hemorrhoids, LH may be preferred over LigaSure due to greater patient comfort, higher satisfaction rates, and fewer early complications. However, close follow-up of these patients is important due to the higher incidence of late complications with LH.

В обзоре литературы представлено историческое развитие хирургических методов лечения геморроидальной болезни. Приводятся примеры наиболее эффективных современных малоинвазивных методик, анализируется опыт их применения, результаты различных комбинаций этих методик. Оценивается эффективность предлагаемых методов в зависимости от технической сложности применения, клинических ограничений, наличия осложнений, рецидивов заболевания и влияния на качество жизни пациентов. Также оценивается эволюция малоинвазивных методов лечения геморроя с течением времени на основании полученных ранее результатов лечения. Особое внимание уделено малоинвазивным методикам с прошиванием сосудистой ножки геморроидального узла, мукопексии и применению лазерных методик лечения геморроидальной болезни при 3-й и 4-й стадиях заболевания. Проанализированы печатные работы, посвященные физическим процессам, возникающим при лазерном воздействии на ткани в зависимости от длины волны применяемых лазерных установок. Также прослежена тенденция к комбинированию различных изолированных методик лечения геморроидальной болезни с целью повышения результата лечения пациентов с хроническим геморроем. The literature review presents the historical development of surgical methods for the treatment of hemorrhoids. Examples of the most effective modern minimally invasive techniques are given, the experience of their application, the results of various combinations of these techniques are analyzed. The effectiveness of the proposed methods is evaluated depending on the technical complexity of the application, clinical limitations, the presence of complications, relapses of the disease and the impact on the quality of life of patients. Special attention is paid to minimally invasive techniques with the residence of the vascular pedicle of the hemorrhoid node, mucopexy and the use of laser techniques for the treatment of hemorrhoidal disease at stages 3 and 4 of the disease. The printed works devoted to the physical processes that occur during laser exposure to tissues, depending on the wavelength of the laser installations used, are analyzed. There is also a tendency to combine various isolated methods of treatment of hemorrhoidal disease in order to improve the outcome of treatment of patients with chronic hemorrhoids.

Introduction Haemorrhoidal disease is one of the most common nowadays. It is often associated with a sedentary lifestyle. The leading cause of its development is also a functional disorder of the intestine and chronic constipation. To date, there is a steady growth rate of this disease, leading to its “rejuvenation”. The current stage of development indicates the need for further improvement of surgical treatment and optimisation of patient management methods and the creation of uniform standards of care for this contingent of patients. Aim To evaluate the clinical effectiveness of the use of platelet-rich plasma therapy and the biologically active substance “ozoyl” in the treatment of haemorrhoidal disease. Material and methods The main group included 100 patients with chronic haemorrhoids who were operated on in the period from March 2021 to March 2022. For this group, autoplasma was used during surgery, and an ozoyl-based drug in the postoperative period. The remaining 100 participants of this study, assigned to the control group, underwent a conventional haemorrhoidectomy operation and standard patient management using a hydrophilic ointment based on chloramphenicol. Results After the conducted clinical studies, it was established that in the main group, the pain syndrome decreased by about 30%, considering the period from the first day of the postoperative period compared to the control group. The postoperative wound healed in the main group in the third week after the operation, unlike the control group, in which this event was noted in the fourth week. The patients did not complain during the examination 3 months later. Conclusions This study is of practical significance because haemorrhoidal disease today has a high prevalence, and an integrated approach is required for the treatment of such patients. Ozoyl is a powerful cell and tissue repairer.
