Laser goniopuncture after deep sclerectomy: incidence, long-term outcomes and risk factors for failure

Abstract: AimsTo report the incidence, risk factors and long-term outcomes of laser goniopuncture (LGP) in patients with previous deep sclerectomy (DS).MethodsRetrospective cohort study of 1765 eyes (1385 patients) undergoing DS with or without cataract surgery between 2001 and 2020 in two UK institutions. Kaplan-Meier was used to estimate LGP incidence. DS success after LGP was calculated for criteria A, B, and C defined as intraocular pressure (IOP) of ≤18, ≤15 and ≤12 mm Hg with 20%, 25% and 30% reduction, respective… Show more

“…Most importantly, all eyes requiring additional IOP-lowering surgery showed bleb fibrosis before DEX-I injections, highlighting the need for careful examination before indicating the intravitreal treatment. Moreover, it should be noted that most patients (85.7%) who had NPDS in the present cohort also underwent goniopuncture during follow-up, as it has been shown to decrease IOP significantly for at least 5 years [19].…”

“…While RCT emulation efforts build confidence in the insights derived from observational studies, expansion of future studies to include more generalizable patients would be valuable. 21 …”

“…Given the observational and retrospective nature of the IRIS Registry, we cannot rule out unobserved variables being associated with the treatment group and impacting our treatment effect estimates, especially if such variables were related to VA progression and differed between treatment groups. 21 Furthermore, the use of the transportability weight adjustment is based on both an exchangeability and positivity assumption. 7 , 13 For the latter, the positivity assumption in our study means that the probability of being included in the IRIS Registry population must be greater than zero for participants in all strata of our transportability weight adjustment model in the VIEW RCT.…”

“…One large retrospective study found a rate of 33% at 1 year and 63% at 5 years. 87 The main complication of goniopuncture is iris prolapse. Iridoplasty, pilocarpine and anterior placement of the goniopuncture are all strategies to reduce this.…”
