Laryngeal Reinnervation Using Ansa Cervicalis for Thyroid Surgery-Related Unilateral Vocal Fold Paralysis: A Long-Term Outcome Analysis of 237 Cases

Abstract: ObjectiveTo evaluate the long-term efficacy of delayed laryngeal reinnervation using the main branch of the ansa cervicalis in treatment of unilateral vocal fold paralysis (UVFP) caused by thyroid surgery.Summary of Background DataUVFP remains a serious complication of thyroid surgery. Up to now, a completely satisfactory surgical treatment of UVFP has been elusive.MethodsFrom Jan. 1996 to Jan. 2008, a total of 237 UVFP patients who underwent ansa cervicalis main branch-to-recurrent laryngeal nerve (RLN) anast… Show more

“…The most common technique reported is nerve-to-nerve anastomosis employing ansacervicalis as the donor nerve as described by Crumley (ansa-RLN) (14). Wang et al (15) had Editorial improved the technique by using the main branch of the ansa cervicalis that has more axons to regenerate to the injured RLN instead of ansa hypoglossi to sternohyoid, and they described details of the surgery for a successful reinnervation.…”

“…Recent treatment methods include thyroplasty, laser arytenoidectomy and cordectomy. The recovery success of the RLN depends on early diagnosis and early exploration of the nerve [77][78][79].The treatment of SLN injury includes voice therapy, type 1 or 4 thyroplasty and reinnervation using nerve muscle pedicle technique. There are no studies which extensively investigated the success of these therapeutic methods for SLN injury [80][81][82][83].…”

“…But this operative standard still results in a 4% to 8% incidence of initial postoperative RLN paralysis; with continued follow up, the RLN frequently recovers function, resulting in a permanent paralysis rate of 1% to 2% [4][5][6][7]. The most common laryngoscopic findings in unilateral vocal fold paralysis beyond vocal fold motion impairment include bowing, incomplete glottal closure, and phase asymmetry on videostroboscopy [14].The position of the vocal fold (eg, paramedian vs lateral) does not necessarily clarify the location of the lesion along the neurologic pathway from brain to motion of vocal fold [15] II.…”
