INTRODUÇÃO: A Doença do Refluxo Gastroesofágico (DRGE) é a doença digestiva mais prevalente da atualidade e, recentemente, tem sido implicada em uma gama de alterações do seguimento laringofaríngeo (RLF). No entanto, pouco se sabe dos mecanismos fisiopatológicos destas manifestações supraesofágicas da DRGE. Os achados clínicos contraditórios e recentes pesquisas sugerem haver deficiências na capacidade de defesa deste seguimento. Uma das principais responsáveis pela homeostase da mucosa oral e do trato digestivo é a saliva com seu conteúdo orgânico e inorgânico. Tanto alterações do pH quanto do volume salivar já foram correlacionados com os sintomas e sinais sugestivos da DRGE e RLF. Estudo recente de nossa autoria demonstra diminuição estatisticamente significante do pH salivar de indivíduos com RLF quando comparado a controles sem a doença. Outro estudo constatou correlação entre a redução do volume X pH da saliva em indivíduos com DRGE, estando esta redução diretamente relacionada aos níveis de pH esofágico constatados durante pH-metria esofágica de 24 horas. OBJETIVOS: Avaliar como se comportam o pH e volume da saliva em um mesmo indivíduo com DRGE e RLF antes e após o tratamento clínico. MATERIAL E MÉTODO: Vinte e três pacientes com RLF tiveram o pH e volume da saliva total testados antes e após receberem tratamento com droga bloqueadora de bomba de prótons durante 12 semanas. RESULTADOS: Houve uma diferença estatisticamente significante (p<0,001) entre o pH da saliva antes e após o tratamento, estando este maior após o controle clínico da doença. O volume de saliva no paciente tratado foi significativamente maior do que no paciente pré-tratamento (p=0.009). DISCUSSÃO: Os achados sugerem que o pH salivar é influenciado pela presença de refluxo gastroduodenal à região laringofaríngea. Caso estudos futuros com populações maiores realmente comprovem esta correlação, poderemos cogitar a possibilidade de usar a mensuração do pH salivar, que é feita de forma rápida e não invasiva, como um meio de diagnosticar e avaliar o comportamento e controle do Refluxo Laringofaríngeo.

Objectives: This study aims to investigate the effects of reflux treatment in voice disorders in laryngopharyngeal reflux (LPR) patients using acoustic analyses and the relationship between scoring systems. Patients and Methods:A total of 84 LPR patients (18 males, 66 females; mean age 43.1±11.3 years; range 18 to 73 years) were evaluated using reflux symptom index (RSI), reflux findings score (RFS), videolaryngostroboscopic examination, and acoustic analysis with Dr. Speech 4 before LPR treatment and at the first and third months after treatment.Results: Maximum phonation time, fundamental frequency (F0), jitter, and shimmer scores did not show any statistically significant alteration at the posttreatment period according to pretreatment scores (p>0.05). However, the alteration in Harmonics-to-Noise ratio and Signals-to-Noise ratio scores were statistically significant (p=0.017 and p=0.003, respectively). Reflux symptom index results showed significant positive correlation with F0 at the pretreatment, and at posttreatment first and third month evaluations (Spearman's rank correlation coefficient [rho]=0.246, p=0.024; rho=0.300, p=0.006; rho=0.305, p=0.005, respectively). Conclusion:The relationship between the parameters of acoustic analysis and RSI and RFS values seems to be controversial for diagnosis and follow-up of LPR patients, requiring further investigations.Keywords: Laryngopharyngeal reflux; reflux scoring system; voice analysis. Resumo / SummaryApesar do grande entusiasmo despertado pelo avanço dos conceitos definidores do conjunto sintomático denominado refluxo laringofaríngeo (RLF), ainda é muito difícil, para o otorrinolaringologista, estabelecer com segurança se esta síndrome poderá ser considerada no futuro como uma doença semelhante ao refluxo gastroesofágico 1 . A nosso ver, os sinais inflamatórios no segmento laringofaríngeo encontrados em alguns pacientes que apresentam phmetria com presença de pH abaixo de 4 na sonda proximal podem ser mimificados por outras situações que agridem a região, não sendo, portanto, patognomônicos de RLF. As condições de fluxo salivar, seu volume, clearence e alterações das condições eletrolíticas da saliva podem influenciar na capacidade de proteção da mucosa regional. Objetivo: Este estudo teve como principal objetivo observar a relação entre os sintomas laringofaríngeos de inflamação e o pH e volume salivares. Forma de Estudo: Observacional coorte com corte transversal. Material e Método: Foram estudados 59 sujeitos com idades variando de 24 a 76 anos, com média 50,5 anos, sendo 44 mulheres e 15 homens. Todos os pacientes responderam a questionário sobre sintomas laringofaríngeos e tiveram sua saliva coletada e seu volume e pH mensurados. Resultados: O volume médio de saliva coletado no total de pacientes foi 4,3 ml, com o mínimo de 1,5 e um máximo de 7,5. O pH médio foi de 7,1, se estendendo de 6 a 8. Do total de sintomas apresentados, 31 pacientes apresentaram disfonia, 39 apresentaram pigarro, 2 dispnéia, 24 halitose, 4 caseo, 4 amigdalite de repetição, 6 problemas dento-gengivais, 9 aftas, 6 xerostomia, 12 glossodínia, 36 globus pharingeus, 2 odinofagia, 16 tosse e 8 disfagia. As diversas correlações entre pH, volume salivar e sintomas foram observadas mostrando, em alguns casos, forte correlação positiva ou negativa. Conclusão: O pH salivar, na dependência do volume salivar, pode ter forte interferência na sintomatologia faringolaríngea.
