Larger biopsies evaluation of transient elastography for detecting advanced fibrosis in patients with compensated chronic hepatitis B

Abstract: Inadequate sample study would underestimate the efficiency of TE on detecting advanced fibrosis. With ALT 2 × ULN stratified cutoffs, TE determined nearly 80% of patients with normal bilirubin as AF or non-AF and obviated them from liver biopsies.

“…q Fibrosis could differentiate between Ishak stages 5 and 6; with the potential to detect the changes of histological patterns in cirrhosis progression or regression through the quantitative classification of different collagen patterns. In future studies correlating with or complementing other clinical markers such as FibroScan® [32] and hepatic vein pressure gradient (HVPG) [33] or complications of cirrhosis, we could further develop q Fibrosis into a tool to aid monitoring the cirrhosis dynamics. Last but not least, since experienced hepatopathologists are a rare breed in most setups, q Fibrosis might act as a valuable aid to pathologists to produce consistent staging of liver fibrosis; as well as to provide on-site expert consultation to the non-expert pathologists.…”

“…Liver stiffness measurement (LSM) by FibroScan has been shown to be an accurate predictor of liver fibrosis in a variety of chronic liver diseases including CHB 6,7 . Previous studies showed a good correlation between LSM and liver fibrosis, with the area under the receiver–operator curve more than 0.90 for METAVIR stage 3 or greater fibrosis in CHB 6,8–10 …”

“…And that may explain the reason why TE value was not associated with time since Fontan surgery is with only P value of 0.06. Study has suggested that grouping patients according to ALT level is necessary for TE detection [8]. Although data from the study indicated that TE was efficacious for detecting liver fibrosis after Fontan surgery, stratifying patients according to ALT levels and comparing with control group with a similar ALT level may be much more convincing and interesting.…”
