Large-Scale Long-Term Follow-Up Study of Japanese Patients With Non-Alcoholic Fatty Liver Disease for the Onset of Hepatocellular Carcinoma

Abstract: This retrospective study indicates that the annual incidence rate of HCC among Japanese NAFLD patients is low. Elderly NAFLD patients with diabetes, elevated serum AST, and especially thrombocytopenia (suggested to be associated with advanced liver fibrosis) should be monitored carefully during follow-up that includes using the APRI to ensure early diagnosis and treatment of HCC.

“…The presence of thrombocytopenia in HCC patients probably reflects the severity of the portal hypertension which is secondary to the cirrhosis in which HCC usually develops, as has been noted previously [11][12][13][14][15] . To our knowledge, an association between thrombocytopenia and HCC size has not previously been reported.…”

“…A low HCC incidence of 0.25% was observed in a retrospective cohort study in a large sample of NAFLD patients diagnosed by ultrasound (16 of 6,508 NAFLD patients) [25] (HR 8.20, 95% CI 1.09-9.50, p=0.035) were independent risk factors for HCC. Hepatocellular carcinoma was associated with signi cant brosis assessed by the AST to platelet ratio index (APRI) (equivalent to stage 3-4 NASH, HR 25.03, 95% CI 9.02-69.52, p<0.001) but not by the BARD score [25].…”

“…El-Serag és mtsai szerint a diabetes 10 év alatt kétszeresre növeli meg a HCC kockázatát [31], míg japán szerzők azt közölték, hogy NAFLD-betegeknél diabetes esetén ez a kockázat több mint háromszoros [32].…”

“…A betegségspecifikus mortalitás legerősebb prediktorának a fibrosis bizonyult [15]. Az előrehaladott fibrosisra/cirrhosisra utaló thrombocytopenia (<150 000 G/l) is magas HCC-kockázatot (HR: 7,14) képvisel NASH-betegekben [32]. Egyébként amíg NASH-cirrhosisban évente 2,6%-ban, HCV-cirrhosisban 4,0%-ban alakul ki májrák [33].…”
