Large‐Scale Evaluation of Enzyme‐Linked Immunospot Assay and Skin Test for Diagnosis ofMycobacterium tuberculosisInfection against a Gradient of Exposure in The Gambia

Abstract: The purified protein derivative (PPD) skin test for Mycobacterium tuberculosis infection lacks specificity. We assessed 2 more specific M. tuberculosis antigens (ESAT-6 and CFP-10) by enzyme-linked immunospot assay (ELISPOT) compared with PPD by ELISPOT and skin test in The Gambia. Of 735 household contacts of 130 sputum smear-positive tuberculosis cases, 476 (65%) tested positive by PPD ELISPOT, 300 (41%) tested positive by PPD skin test, and 218 (30%) tested positive by ESAT-6/CFP-10 ELISPOT. Only 15 (2%) ha… Show more

“…Five 153,166,177,178,258 studies used assays based on ELISPOT (n = 1461), seven 154,155,158,[255][256][257]259 used whole blood interferon-␥ ELISA (n = 2516) and one 260 used an unspecified interferon-␥ assay [peripheral blood mononuclear cells (PBMC) ELISA] (n = 58), including a total of 4035 subjects (Figure 25, Appendix 27). Four studies provided no information on TB exposure.…”

“…(35,(50)(51)(52) In the ELISPOT, IFN-γ molecules secreted by cells of the peripheral blood bind specifically to anti-IFN-γ monoclonal antibodies previously immobilized on a plate, avoiding the problem of cytokine consumption during culture, thus increasing the sensitivity of the ELISPOT in comparison with the ELISA.The T SPOT.TB ® assay (Oxford Immunotec, Oxon, UK) uses ESAT-6 and CFP-10 as specific antigens for the stimulation of blood T lymphocytes. This assay is awaiting FDA approval.…”

“…Most studies demonstrate a concordance ranging from modest to high (60-80%) between the two tests. (52)(53) Various studies suggest that the tests directed at the detection of RD1 antigen-induced IFN-γ production surpass the PPD test in terms of the following characteristics: specificity; correlation with indirect measurements of exposure to M. tuberculosis; lower cross reactivity with BCG vaccination or with infection caused by environmental mycobacteria; and faster laboratory test results. (35,(46)(47) Due to the lack of a standard determinant in the diagnosis of TB, it is impossible to define with precision the sensitivity and specificity of the tests that can quantify IFN-γ production for the diagnosis of latent infection.…”
