Pulmonary Hamartomas are benign lung tumors, containing cartilage, connective tissue, fat and usually appear as peripheral lesions. They are 2-4 cm in size, frequently asymptomatic, comprising of 5–8% of all solitary pulmonary nodules. The peak incidence occurs in 40-70 years of age with a male preponderance. We describe a unique case of centrally located, giant chondroid hamartoma, in the background of pulmonary hemosiderosis. Patient presented with cough, shortness of breath and left sided chest pain, masquerading as pleural effusion, lung cancer and left sided chest pain mimicking myocardial disease. Chest x-ray raised the suspicion of pleural effusion but ultrasound and CT-scan indicated a benign calcified central mass. A presumptive diagnosis of cartilage-containing benign mass was made on ultrasound-guided percutaneous biopsy, while surgical resection followed by histopathology revealed a chondroid hamartoma. No recurrence was noted on follow-up. Chondroid hamartoma can be large, symptomatic and requires modern modalities for diagnosis while its association with pulmonary hemosiderosis, still requires to be investigated.

Pneumonia is an infection of the lungs that can result from various etiologies, including bronchial obstruction. It is estimated that 5.4% of community-acquired pneumonia occurs as a result of an endobronchial obstruction, classifying them as post-obstructive pneumonia. Pulmonary hamartomas are benign and exceedingly rare tumors. These hamartomas are usually asymptomatic and found incidentally on imaging, however, they can cause patients to develop post-obstructive pneumonia. We present a 40-year-old female with cough, fatigue, and recurrent right lower lobe pneumonia. Upon workup with bronchoscopy and biopsy, she was subsequently found to have an endobronchial hamartoma resulting in recurrent pneumonia in the same location. We are happy to report that the patient had a resection of the mass, as well as of the affected lung lobe, and has been pneumonia-free for five months. We hope to encourage a greater index of suspicion for endobronchial masses, including rare tumors, when a patient presents with recurrent pneumonia in the same location.

Akciğerin benign lezyonu olan pulmoner hamartom nadir bir lezyondur ve soliter pulmoner nodüllerin %6'sını oluşturur. Çalışmada, hastanemizde tanı alan, 10 pulmoner hamartom olgusunun klinik, radyolojik ve patolojik özellikleri incelenmiştir. Gereç ve Yöntemler: Ocak 2006 ile Aralık 2016 tarihleri arasında hastane dosya arşivi ve bilgisayar verileri retrospektif olarak incelendi. Bulgular: Olguların yaş ortalaması 52,3 yıl (33-67yıl) olup, altı olgu kadın, dört olgu erkekti. Yedi olgu semptomatik olup bir olguya eş zamanlı akciğer adenoskuamöz karsinomu eşlik etmekte idi. Lezyonların çoğunluğu (%60) sol akciğerde yerleşimli ve ortalama çapı 3,12 cm (1-6 cm) idi. Tüm olgular periferik yerleşimliydi. Yalnızca bir olguda (%16,6) popcorn kalsifikasyon izlendi. Tüm olgulara wedge rezeksiyon ve kitle enükleasyonu içeren akciğeri koruyucu cerrahi uygulandı. Eksizyon sonrası, ortalama 36,23 ay (25 gün-113 ay) takip süresi boyunca nüks veya malignite gelişimi izlenmedi ve pulmoner hamartom nedenli ölüm olmadı. Sonuç: Akciğer kanseri ayırıcı tanısı nedeniyle pulmoner hamartom önemlidir. Radyolojik olarak ancak %34-50 oranında tanı verilebildiği için multidisipliner davranmanın gerekli olduğu, klinik ve radyolojik bulgular eşliğinde ince iğne aspirasyon veya tru-cut biyopsi kullanılarak tanı konulabileceği ve cerrahi uygulanmaksızın güvenle takip edilebileceği unutulmamalıdır. Cerrahi sonrası ise takip gerekli değildir.
