Laparoscopy impacts outcomes favorably following colectomy for ulcerative colitis: a critical analysis of the ACS-NSQIP database

Abstract: Results from a large nationwide database demonstrate that a laparoscopic approach was utilized in an increasing number of UC patients undergoing colectomy and was associated with lower morbidity and mortality, even in more complex procedures, such as TAC and IPAA.

“…Of those, 47 were excluded because they did not report original research (n=1), were not population-based (n=13), did not report postoperative mortality data (n=20), included surgeries performed before 1990 (n=3), did not differentiate between CD and UC patients (n=6), used the same data source as a more up-to-date manuscript (n=3) 23,24,25 , and were restricted to a pediatric population (n=1). Of the 21 remaining articles, 9 reported on UC only 5,11,12,26-31 , 4 reported on CD only [32][33][34][35] , and 8 reported on both UC and CD.…”

“…Similar observations reporting benefits of laparoscopic colectomy with regards to surgical site infection and bowel recovery have been made in large population-based studies, for a variety of diseases. [18][19][20] Anastomotic leakage is one of the most feared complications of bowel resection in elective and emergent settings. In our series, 2 patients in each group suffered from anastomotic leak.…”

“…A minimálisan invazív technika hazánkban is egyre nagyobb teret nyer, amely módszer az IBD kezelésében is széles körben, biztonsággal alkalmazható eljárás [6,7,8]. Tanulmányunkat három nagy részre (perioperatív időszak, műtét utáni komplikációk és életminőség) bontva, retrospektív módon vizsgáltuk a …”

“…Két amerikai tanulmány hasonlóan kedvező eredményeket közölt ápolási idő, vér igény, kozmetikai eredmény, sebfertőzés terén sürgő-séggel végzett laparoszkópos szubtotális colectomiák kapcsán [23,24]. Egy nagy nemzetközi adatbázis adatai alapján a minimálisan invazív betegcsoport kedvezőbb eredményekkel bír morbiditás és mortalitás szempontjá-ból [6]. Míg egy 2013-ban megjelent randomizált tanulmány a laparoszkópos módszernek csupán a kozmetikai előnyét igazolta [25].…”
