Laparoscopically assisted vaginal and abdominal hysterectomy: comparison of postoperative pain, fatigue and systemic response. A case-control study

Abstract: Follow-up of postoperative pain scores during mobilization and fatigue levels might be an easy tool for the evaluation of postoperative recovery. Using an identical anaesthetic technique, the neuroendocrine response was of the same magnitude after both types of surgery.

“…In the case of hysterectomy, Ribeiro et al and Labib et al found that laparoscopically assisted vaginal hysterectomy was associated with decreased release of IL-6 [37,49], and similar results were obtained by Malik et al and HarkkiSiren et al [25,41]. In contrast, Rorarius et al and Ellstr} om et al observed similar levels of IL-6 after both laparoscopic and open-abdomen procedures [18,52].…”

“…In patients with (6)(7)(8)(9) or without (10)(11)(12)(13) endometrial cancer, publications were able to demonstrate a benefit of laparoscopic hysterectomy compared to an open hysterectomy via laparotomy with regard to postoperative morbidity. These publications compared laparoscopic-assisted vaginal hysterectomy (LAVH) with total abdominal hysterectomy (TAH).…”

“…The ®nding that interventions such as laparoscopic surgery and glucocorticoid provision appear to have most effect at d0±d1 and d2±d7 supports the suggestion that some acute aspect of the surgical stress response may play an important role during early recovery 23 . For example, both interventions dampen cytokine expression following surgery 19,66 , a phenomenon that has recently been shown to correlate with fatigue 85 . As increased analgesia also reduces early fatigue, it is possible that sleep disturbance due to pain may also be a contributing factor 23,86 .…”
