Laparoscopic vs open approach for Nissen fundoplication

Abstract: The open and laparoscopic approaches for the Nissen fundoplication are equally effective in controlling GERD. The open approach is associated with a significantly higher rate of wound and respiratory complications and, at early stages, an increased rate of postprandial epigastric fullness and abdominal bloating. The dysphagia rate is similar with both methods.

“…The results of several nonrandomized trials did not confirm this concept [16,33,34]. None of these three randomized trials of laparoscopic fundoplication techniques stratified according to preoperative esophageal motility, so valid information about the tailoring concept is not available [35,36]. The present study exceeds all other randomized investigations in terms of numbers of study population.…”

“…Two years after the surgery, GIQLI scores were significantly improved, compared to preoperative data, but did not differ between the laparoscopic and open groups. In a similar study by Chrysos et al [20], patients were given an ad hoc questionnaire after laparoscopic and open Nissen fundoplication (1b). Follow-up at 12 months included 106 patients.…”

“…Laparoscopic fundoplication is at least as safe and as effective as its open counterpart, enabling reduced morbidity, shorter hospital stay, and reduced sick leave [5,13,17,20,22,23]. The complication rate for this procedure has been shown to decrease with surgeon experience [14,18,30,31].…”
