Laparoscopic versus open transhiatal oesophagectomy for oesophageal cancer

Abstract: There are currently no randomised controlled trials comparing laparoscopic with open transhiatal oesophagectomy for patients with oesophageal cancers. In observational studies, laparoscopic transhiatal oesophagectomy is associated with fewer overall complications and shorter hospital stays than open transhiatal oesophagectomy. However, this association is unlikely to be causal. There is currently no information to determine a causal association in the differences between the two surgical approaches. Randomised… Show more

“…Despite the laparoscopic approach being described many years ago, it has been poorly adopted due to the paucity of strong evidence advocating its use. There are observational studies available demonstrating noninferiority between laparoscopic and open oesophagectomy for oesophageal cancer [26]. The lack of observational studies or trials involving acute oesophageal perforations is due to the low incidence of cases encountered to demonstrate non-inferiority for laparoscopic repair.…”

“…Some have advocated its use in distal esophageal cancers (11), while others have reported poor visibly leading to inadequate lymph node dissection and difficulty with hemostasis (12). Overall, laparoscopic transhiatal esophagectomy is associated with fewer complications and shorter hospital stays (13). Nevertheless, there are currently no known randomized studies strictly comparing a minimally invasive transhiatal esophagectomy to conventional OE in the setting of esophageal cancer.…”

“…Ученые из Великобритании (2016) продолжают изучать отличия между лапароскопической операцией и открытой трансхиатальной эзофагэктомией у пациентов с РП [15]. Как показали исследования, лапароскопическая трансхиатальная эзофагэктомия связана с меньшим количеством общих осложнений и более коротким сроком пребывания в больнице, чем открытая трансхиатальная эзофагэктомия.…”
