Laparoscopic ventral hernia repair: is there an optimal mesh fixation technique? A systematic review

Abstract: None of the currently available mesh fixation techniques used for LVHR was found to be superior in preventing hernia recurrence as well as in reducing abdominal wall pain. The pain reported was remarkably high with all different fixation devices. Further research to develop solid and atraumatic fixation devices is warranted.

“…Therefore, Carbajo and Eid generally recommended an intra-abdominal suture [32,33]. Although suture tensile strength is greater than that of tacks, and despite numerous anecdotal reports of hernia recurrence secondary to suture failure or omission, the existing literature does not show superiority of one mesh fixation technique over the other with regards to recurrence rates, whereas infection rates increase when transfascial sutures are used [34,35].…”

“…Studies analyzing various mesh fi xation methods include, most recently, a meta-analysis looking at 25 retrospective and prospective trials comparing fi xation with tacks and sutures vs. tacks alone vs. nonabsorbable sutures alone determined an overall hernia recurrence rate of 2.7% (95% CI [1.9-3.4%]) and no statistically signifi cant difference in recurrence rates between the compared groups [ 33 ]. Furthermore, while pain in both the early postoperative period (<4 weeks) and long term (>1 month) were increased with any type of invasive mesh fi xation, sutures were associated with more pain compared to tacks.…”
