Laparoscopic treatment of colovesical fistulas: technique and review of the literature

Abstract: Colovesical fistula is an uncommon complication of diverticulitis. We present our technique of a laparoscopic approach for treatment of vesicosigmoid fistulas and review the available published literature. We believe that a laparoscopic approach is a feasible and advantageous alternative for the treatment of colovesical fistulas, with low morbidity and short hospital stay.

“…8 Tsivian et al described another successful laparoscopic resection with a shorter operating time (230 minutes). 9 In the current study, operating times were significantly lower (median 135 minutes) and compared favorably with those reported for laparoscopic management of complicated diverticular disease, diverticular fistulas, and mixed fistulas (see Table 1). Shorter operating times likely reflect the surgeon's experience and the evolution of surgical technology.…”

“…[4][5][6][7] A review of the literature indicated very few studies on the laparoscopic management of diverticular colovesical fistulas exist. 4,8,9 The outcomes of recent studies are difficult to interpret as many have incorporated both recurrent uncomplicated and complicated disease, have not differentiated between colovesical fistulas and other diverticular fistulas, [10][11][12][13][14][15] or have included fistulas of various aetiologies. [16][17][18][19][20] Other studies have not used a total laparoscopic approach but have performed technically challenging aspects with a resection-facilitated or hand-assisted technique.…”

“…Intravenous urography does not delineate the fistula unless the patient has severe outflow obstruction [43]. Barium enema imaging rarely reveals a fistula but can demonstrate the nature and extent of colonic disease [3]; the reported sensibility rate of barium enema is approximately 20% [4-6]-35% [11,44]. Radiography of centrifuged urine samples obtained immediately after a barium enema (Bourne test) may enhance the yield of the enema by showing barium in the urinary sediment [3].…”
