Laparoscopic subtotal splenectomy in hereditary spherocytosis

Abstract: The paper by Sanjeev Dutta et al. [1] about the laparoscopic approach to partial splenectomy in children with hereditary spherocytosis states that they have developed a novel laparoscopic partial splenectomy technique that combines the benefits of partial splenectomy with those of a laparoscopic approach. This technique, however, is not new, as erroneously stated by the authors. The technique was first published in 2003 [2] (the paper, with an English abstract, is listed by Medline). Subsequent results were pr… Show more

“…Laparoscopic subtotal splenectomy for hereditary spherocytosis was described in two cases in 2003 [24], however, until now, paediatric surgeons had to choose between the advantages of the laparoscopic approach and those of partial splenectomy by laparotomy. The surgical procedure we propose in our report can free surgeons from such a choice, since it associates the benefits of both the laparoscopic and partial approaches.…”

“…This includes disorders like non-parasitic splenic cysts; intra-splenic pancreatic cysts; partial infarctions; benign tumours like hamartomas, fibromas or haemangiomas; metastases, usually of gynaecologic origin; splenic lesions of uncertain histology; and staging laparoscopies for lymphomas. Also, partial or even subtotal splenectomies can be effective alternatives to total splenectomies in patients suffering from hereditary spherocytosis [8,9] and in patients following traumatic or surgical splenic lesions [10]. However, laparoscopic splenectomies in general and laparoscopic partial splenectomies in particular remain to be a surgical challenge.…”
