Laparoscopic splenectomy with peginterferon and ribavirin therapy for patients with hepatitis C virus cirrhosis and hypersplenism

Abstract: Laparoscopic splenectomy allows patients with HCV cirrhosis and hypersplenism to receive full-dose peg-IFN + RIB therapy. Patients with HCV, genotype 2 or 1b and a low viral load, and hypersplenism may be good candidates for Lap-Sp.

“…As a background factor, the use of splenectomy in patients with liver cirrhosis is reported to improve the T1/T2 balance and achieve antitumor effects (20). Since splenectomy also significantly improves leukopenia and neutropenia, the use of splenectomy in cirrhotic patients may improve the immune function (21).…”

“…Open or laparoscopic splenectomy seems to be effective for hypersplenism. However, splenectomy might induce more serious complications, such as portal vein thrombosis, 16 pancreatic fistula, 7 and post-splenectomy infections, which can be overwhelming with potentially rapidly fatal septicemia. 17 Recently, advanced skills and anti-pneumococcal vaccination are gradually resolving these problems.…”

“…6,7,12 In this way, control of hypersplenism may be used as a "bridging therapy" for patients with decompensated cirrhosis awaiting liver transplantation. A less invasive and effective treatment is desirable for improving the prognosis in cirrhotic patients, and the present experimental studies by Liver fibrosis occurs in response to almost all causes of chronic liver insults, and the initiation of its deposition imposes an important phase in chronic liver disease.…”
