Gastric bypass surgeries for obesity are hypothesized to produce weight loss and improve diabetes by multiple mechanisms including gastric restriction and lower gut stimulation.The surgeries typically involve rearrangement of the fore-gut, hind-gut or both. We compared the effects of foregut (Sleeve Gastrectomy, SG), hindgut (Ileal Transposition; IT), both (SG+IT), or sham manipulations, on food intake, body weight and glucose tolerance in male SD rats. SG, IT and SGIT surgeries produced transient reduction in food intake and weight gain but improvements in glucose tolerance. SG, IT and SGIT surgeries increased plasma GLP-1 concentrations; IT and SGIT increased PYY concentrations. The protein abundance of key markers of glucose metabolism (e.g. GLUT4, AMPK, PKA, IRS-1) in muscle and adipose tissue were increased following SG, IT, or SGIT. Improvement in glycemic control following these surgeries appears to be independent of changes in weight and is likely due to enhanced lower gut stimulation.iii Acknowledgment

Morbid obesity is a worldwide health concern that compromises life quality and health status of obese human subjects. Bariatric surgery for treating morbid obesity remains as one of the best alternatives to promote excess weight loss and to reduce comorbidities. We have not found studies reporting nutrients and energy balance considering digestibility trials in humans following surgery. The purpose of this study was to determine protein, lipid, fiber, energy, calcium, and phosphorous digestibility in a swine model that underwent ileal transposition (IT), sleeve gastrectomy with ileal transposition (SGIT), Roux-en-Y gastric bypass (RYGBP), and with sham operated animals (SHAM). Thirty-two pigs were randomly assigned to four laparoscopic procedures:, and Sham-operated pigs (n ¼ 8). From day 0 postsurgery to 130, pigs were weighed monthly to determine live weight and weight gain was calculated for each month postsurgery until day 130. Food intake in a metabolic weight basis was calculated by measuring ad libitum food intake at day 130. Swine were fitted into metabolic crates to determine digestibility coefficients of dry matter, protein, fat, fiber, ash, energy, calcium, and phosphorous from day 130. A one-way ANOVA and Student-Newman-Keuls were used to detect differences in weight, food intake, and digestibility coefficients. Digestibility values for dry matter, fiber, phosphorus, and energy showed no differences among groups (P > 0.05). However, significant differences (P 0.05) were encountered among groups for fat, protein, ash, and calcium digestibilities. The RYGBP procedure, when applied to the pig model, significantly reduced calcium, fat, and ash digestibility, which did not occur with SGIT or IT procedure, when compared with Sham-operated animals.

Введениенастоящее время сахарный диабет 2 типа (СД2) яв-ляется широко распространенным заболеванием, а эксперты ВОЗ рассматривают сло жившуюся ситуацию как пандемию. В 2010 г. в мире насчитыва-лось 285 млн взрослых больных сахарным диабетом (СД), а в России -3,16 млн. Учитывая динамику заболеваемо-сти и смертности за последние 10 лет, ожидается увеличе-ние заболеваемости СД в мире с 6,4% до 7,7%. При этом к 2030 г. прогнозируется увеличение числа зарегистриро-ванных больных СД до 439 млн человек в мире и до 5,81 млн человек в России [1,2]. Социальную значимость проблемы увеличивает то обстоятельство, что СД приводит к ран-ней инвалидизации и смертности в связи с фатальными сосудистыми осложнениями. Несмотря на проведение адекватной лекарственной терапии, показатели гликемиче-ского контроля (в том числе, содержание гликированного гемоглобина -HbA 1c ) у пациентов с СД зачастую не нор-мализуются, что связано с прогрессирующим нарушением функции β-клеток поджелудочной железы [3].Наряду с консервативной терапией, для лечения СД2 с сопутствующим ожирением сегодня уже используются бариатрические операции. Данное направление хирур-гии, изначальной целью которого являлось снижение массы тела у пациентов с морбидным первичным ожире-нием, начало формироваться в 50-х годах прошлого века. По мере развития бариатрической хирургии были вы-явлены положительные эффекты проводимых операций на сопутствующие ожирению заболевания, а именно, СД2, дислипидемию и артериальную гипертензию.
