Laparoscopic Right Posterior Sectionectomy for Malignant Lesions: An Anatomic Approach

Abstract: Laparoscopic right posterior sectionectomy is technically demanding. A proper inflow and outflow control is mandatory for proper anatomical resection. This surgical principle should not be compromised in the era of laparoscopic hepatectomy.

“…13,24 In some series, average blood loss and conversion rate reached 1500 mL and 23%, respectively. 29 In our early experience of partial robotic hepatectomies for lesions in the posterosuperior segments, the blood loss reached 1500-2000 mL; but it required only a few procedures to reduce blood loss. 30 In a whole series, robotic approach was applied more often than laparoscopic technique for anatomical parenchymalsparing resection of the posterosuperior segments for lesions with large vessels involvement.…”

“…The Morioka guidelines jury expressed their concern on this matter, stating that laparoscopy enables magnified visualization of hence increasing the feasibility of sparing liver parenchyma. Three studies have reported on the feasibility and safety of LRPS [4][5][6]. Table 2 gives an overview of the current literature (series > 10 cases) on LRPS and the applied technique.…”

“…Numerous reports have addressed laparoscopic minor liver resections and left lateral sectionectomies but only few papers have focused on laparoscopic right posterior sectionectomy (LRPS) [4][5][6]. LRPS is a technically challenging procedure.…”
