Laparoscopic resection of the pancreas and review of the literature

Abstract: Although laparoscopic pancreatic resection of selected patients is a feasible and safe procedure in the hands of experienced laparoscopic surgeons, patients must be carefully observed after surgery to avoid serious conditions by pancreatic fistula.

“…Since then, an increasing number of reports on successful laparoscopic resection for pancreatic insulinomas have appeared in the English literature (including online publication). Including the current series, 12 case series [2,6,9,10,13,15,16,20,24,29,33] and 12 case reports [3, 4, 7, 11, 21, 22, 26-28, 30, 31, 34] have documented successful outcomes for 61 of 69 patients who underwent attempted laparoscopic resections for pancreatic insulinomas from 1996 to 2002.…”

“…At the same time, laparoscopic surgical techniques have become well established in recent years, and several reports on laparoscopic distal pancreatectomy for low-grade malignant pancreatic tumors have been published (6)(7)(8)(9)(10)(11). In the Warshaw technique, the postoperative splenic circulation is maintained by the collateral vessels via the short gastric vessels.…”

“…If insulinomas locate near the surface of the gland, simple enucleation may be performed to preserve the pancreatic parenchyma as much as possible. To minimize invasion, laparoscopic enucleations have been used for relatively small tumors (<2 cm in diameter) and for tumors located on the surface of pancreas away from the pancreatic duct (13). In the present case, typical clinical symptoms and the result of ASVS indicating the tumor localization in the head of pancreas, prompted us to pinpoint its localization by performing SSCTA.…”
