Laparoscopic Resection of Retroperitoneal Paragangliomas: A Comparison with Conventional Open Surgical Procedures

Abstract: Laparoscopic procedure is safe and feasible in the treatment of patients with retroperitoneal PGLs. Laparoscopic resection of PGLs from the renal hilum is technically challenging. However, it is possible to safely undertake laparoscopic resection of these tumors by an experienced surgeon.

“…Open exploration and resection is the standard surgical management of PG, however, laparoscopic resection of PG is considered challenging because of the altered anatomic location, dense peritumoral adhesions, high vascularity envelope, proximity to major blood vessels [ 1 , 7 ]. If a tumor is felt to be unresectable at surgery, attempts to reduce its size by chemotherapy or radiation or embolization may be indicated because resection offers the only chance of cure [ 4 ].…”

“…Retroperitoneal paragangliomas are rare tumors, While pheochromocytomas originate in the core of the adrenal glands, extra-adrenal paraganglioma tumors arise from ganglia along the sympathetic and parasympathetic chain. Most of them are functional, with symptoms and signs of catecholamine overproduction, similar to pheochromocytoma except for the variation in the anatomic location The risk of malignancy may be as high as 20%–30% compared with a 10% risk of malignancy for adrenal pheochromocytoma [ 1 ].…”

“…However, several reports have described the safety of laparoscopic surgery for retroperitoneal tumors, indicating that the laparoscopic approach is feasible in experienced hands. In our hospital, laparoscopic surgery is usually performed by an operator who is proficient in retroperitoneal tumor resection.…”

“…Scope of the surgery resulted from the suspicion of the invasion of the structures adjacent to the bladder and proximity of distal parts of ureters and ureteral orifices. In the case of paragangliomas located far from a urinary bladder triangle, partial resection of a urinary bladder should be considered (7) . In the case of pheochromocytoma located on a side wall nearby a ureteral orifice or distal part of the ureter, resection of the leasion may compel ureteral reimplantation into a normal bladder wall.…”
