Laparoscopic Resection of a Left Upper Quadrant Mass Leading to a Surprise Diagnosis

Abstract: Background. Resplenectomy is most commonly done for the treatment of recurrent idiopathic thrombocytopenic purpura (ITP) refractory to medical management due to the regrowth of a missed accessory spleen. Case Report. A 66-year-old male had undergone open splenectomy for traumatic rupture 40 years ago. He presented with a leiomyosarcoma of his leg, which was surgically removed. “…Liu reported a patient who presented with a bleeding Dieulafoy lesion at the lesser curve two days after sleeve gastrectomy, which was managed by endoscopic clipping [ 25 ]. Our two patients with benign lesions of the gastric fundus did well, and in both cases, the LLG was part of a more complex surgery including resection of a left upper quadrant mass [ 20 ] and paraesophageal hernia repair. In the hiatal hernia patient, removal of the fundus polyposis together with esophageal lengthening was achieved with the LLG [ 17 ].…”

“…However, despite advantages over open surgery, the rarity of splenosis has limited a more thorough evaluation of its safety and effectiveness. Our evaluation of previous cases in PubMed, Ovid, and other databases found only 15 reports of laparoscopic ectopic splenectomy, while laparoscopic gastric ectopic splenectomy was rarely reported[ 17 , 18 ].…”
