Laparoscopic Removal of a Persistent Mullerian Duct in a Male: Case Report

Abstract: Sir,Since I first described the laparoscopic management of persistent mullerian duct syndrome (PMDS) in early 1997, there have been only very few publications in this field (3). I am, therefore, very much intrigued by the recent report on the laparoscopic excision of a mullerian remnant in a 15-year-old-boy. The report rekindles debates on this surgical dilemma.Recently, successful laparoscopic orchidopexy in two more cases of PMDS reaffirmed our treatment regime (5).

“…The vas deferens can be particularly in close proximity with the fallopian tubes and the vagina. At times, it can be incorporated into the wall of the vagina [8]. Vandersteen et al (1997) evaluated the risk of retaining müllerian structure versus risk of injury to the vas deferens during excision of the müllerian remnants and concluded that the müllerian remnants should not be excised, as malignancy had not been reported till then [6].…”

“…Current literature on PMDS suggests that the patient is at risk for malignancy developing in the testes and also the mullerian remnant [12][13][14][15][16][17][18]. There is no agreement regarding the removal of the rudimentary mullerian structures.…”
