Laparoscopic release of celiac artery compression syndrome facilitated by laparoscopic ultrasound scanning to confirm restoration of flow

Abstract: A 43-year-old woman presented with symptomatic mesenteric ischemia caused by median arcuate ligament compression of her celiac artery. Magnetic resonance angiography clearly demonstrated stenosis of the proximal celiac artery. She underwent laparoscopic decompression by division of the ligament and excision of the celiac plexus. Laparoscopic Doppler ultrasound scanning demonstrated markedly improved flow in the artery. She was discharged in 15 hours and reported complete resolution of her symptoms at the 3-mon… Show more

“…9 Reports on CACS in a preadult age group have not been found in the literature. In adults, CACS is a well-known disorder, [10][11][12] whereas CA stenosis is more frequent than external compression in this age group. 13 However, arteriosclerotic narrowing is not a rare finding in asymptomatic individuals with generalized peripheral arteriosclerosis.…”

“…Decompression of celiac artery is the general approach to treatment of MALS (11) . Either open surgical (11) or laparoscopic (3,21,22) approach is used. The other interventions include celiac gangliectomy, celiac artery revascularization (aorto-coeliac bypass, patch angioplasty) (11) , percutaneous transluminal angioplasty (PTA) (5,11,23) , robotic ligament release, angioplasty with stenting in recalcitrant cases (24,25) .…”

“…In the treatment of CACS, only PTA and surgical approaches were suggested in the documents reviewed (9,(16)(17)(18). Although in a few reported cases the symptoms were completely relieved after performing PTA; the symptoms of most patients receiving PTA are only relieved for a short time (18,19 (19).…”
