Laparoscopic Nephrectomy: The Experience of the Laparoscopy Working Group of the German Urologic Association

Abstract: Laparoscopic nephrectomy has become a well established procedure in those urology departments focusing on laparoscopy. The indications and results are reproducible at these centers. However, for patients with severe perinephritis (that is renal tuberculosis, xanthogranulomatous nephritis, posttraumatic atrophy) a higher likelihood of open conversion must be considered.

“…The benefit of a less invasive surgical approach to urological disease is based on patient comfort, improved cosmetic results, and shorter convalescence compared to open surgery. 7,8 Renal anomalies, including agenesis, dysplasia, hypoplasia, ectopia, polycystic kidney, and horseshoe kidney, have been reported in SIT. Because of the association between situs inversus and cardiac, pulmonary, and renal anomalies, patient management with situs inversus and urologic disease requires careful preoperative evaluation.…”

“…Rassweiler et al 61 publicaron la experiencia inicial en nefrectomía laparoscópica de 14 centros pertenecientes al grupo de cirugía laparoscó-pica de la Asociación Urológica Alemana, donde se realizaron un total de 482 nefrectomías laparoscópicas. Encontraron que las tasas de complicaciones y conversión, así como el tiempo operatorio, dependieron principalmente de la patología renal.…”

“…[3][4][5] Nevertheless, it seems that laparoscopic procedures have complication rates that are comparable to those of open surgeries. [6][7][8][9] In some laparoscopic series, reintervention was necessary in 0.7% to 1.5% of the cases, 1,10 and it was usually associated with technical difficulties, such as adhesions and anatomic structures displacement, particularly when the patient previously had open surgery. Reoperation is always a stressful condition for patients and surgeons, and it carries the spectrum of medical malpractice issues.…”
