Laparoscopic management of appendicular mucinous cystadenoma, case report

Abstract: HighlightsAppendicular mucinous cystadenoma is a rare benign tumour of the appendix. It should be considered in the differential diagnosis of cystic lesion in right lower quadrant of the abdomen.Care must be taken during the operation not to cause spillage of its content.Laparoscopic surgery in patients with appendicular mucinous cystadenoma is technically challenging but can be done safely by experienced laparoscopic surgeon.

“…Furthermore, mucocele of the appendix can be associated with synchronous colorectal neoplasms in 19-25% of cases (11). Therefore a colonoscopy should be performed before surgery to assess the extent of surgical resection (10,12). A characteristic feature for AM can be found on colonoscopy known as "volcano sign" which is caused by fluctuation of protruded appendiceal ostium according to the respiratory movements (10).…”

“…The specimen should be promptly positioned in an endobag for further exploration and extraction in order to avoid peritoneal dissemination. The surgeon should have a low threshold to convert to open surgery in case complete resection cannot by achieved by laparoscopy or in case of suspected extra organ local involvement [ 10 , 11 ].…”

“…The role of laparoscopy has been debated in the last years, with the surgeons’ increasing confidence. It is usually safe if care is taken not to spill the content of the mucocele, not to rupture the appendix and to put the specimen immediately in a plastic bag [ 11 ].…”
