Laparoscopic intraluminal (intragastric) surgery for early gastric cancer

Abstract: A new laparoscopic operation for the treatment of mucosal or submucosal gastric lesions has been designed and performed on eight patients. In this procedure, all three trocars are placed in the gastric lumen, penetrating both the abdominal and stomach walls in order to perform a laparoscopic removal of gastric lesions. The operation is then carried out in the gastric lumen with currently available laparoscopic instruments and laparoscopic monitoring. The procedure is easy, safe, and feasible for mucosal or sub… Show more

“…A variety of minimally invasive treatments for early gastric cancer have been reported, such as EMR [1][2][3][4], laparoscopic wedge resection [5], and laparoscopic intraluminal surgery [6]. However, these operations do not include regional LN dissection.…”

“…Therefore, it would seem logical to carry out the resection of perigastric LNs, at least for patients with early gastric cancer. In this respect, LADG has higher curability (similar to that of conventional ODG with regional LN dissection) than that of the above-mentioned operations [1][2][3][4][5][6]. However, the usefulness of and indications for LADG still remain unclear.…”

“…(Fig. 2) And Ohashi et al attempted the intragastric mucosal resection (IGMR) for EGC of the stomach located at the posterior wall for the first time [8]. The aim of both procedures was to minimize the extent of gastric resection for the treatment of a malignant disease with a low risk of lymph node metastasis, therefore to reduce the accompanying physiologic side effects by the standard gastrectomy.…”
