Laparoscopic incisional hernia repair: are transfascial sutures necessary? A review of the literature

Abstract: On the basis of this review, a larger overlap of the prosthesis (5 vs 3 cm) is necessary if sutures are not used. If sutures are used, they should be placed no more than 5 cm apart. Prospective randomized trials with and without of transfascial sutures using a consistent biomaterial are necessary to settle this issue.

“…Others recommend the use of both. [6,[23][24][25] Respondents in our survey almost uniformly employ both. In the face of the consensus regarding use of mesh and methods of securing the mesh, we conclude that the lack of consensus regarding the use of the laparoscopic technique for incisional hernia repair is not simply a lack of knowledge of the literature related to hernia repair.…”

“…There are also certain concern about the cosmetic appearance of these transfacial suture puncture sites over the skin of the abdominal wall after surgery. [10] Variable recurrence rate have been reported in the literature with the use of different mesh fixation techniques. [11] In a collective review of 23 studies and 12 comparative studies by LeBlanc et al, mesh fixation with sutures only resulted in the lowest recurrence rate compared to that by tacks alone.…”

“…Minimal acceptable overlap is 3 cm if transfascial sutures are used. 7 Adequate overlap promotes tension-free repair and proper closure of the defect. We used minimum mesh overlap of 4 to 5 cm on all sides.…”
