Laparoscopic complete/partial nephroureterectomy in children in a single institution—nearly a decade of experience: what have we learned?

Abstract: Laparoscopic nephroureterectomy/hemi-nephroureterectomy is well tolerated in children and should be the preferred option. Operative time is acceptable. There are few complications. Patients with MCDK should have an ultrasound before surgery to confirm ongoing presence of renal tissue. In patients with xanthogranulomatous disease, laparoscopy should be avoided.

“…[31] However, a high conversion rate (27%) and longer operative time should be expected in adults [32] and in children. [33] Furthermore, nephrectomised children, especially those with vesico-ureteral reflux (VUR), may experience complications related to persistence of the distal ureteral stump. In our patient, the laparoscopicassisted approach was chosen to remove the distal stone and, at the same time, minimize the residual ureteral stump.…”

“…In an attempt to establish a flowchart for the management of MCDK that answers the existing questions, aligns cost and morbidity, and optimizes the benefits of clinical presentation and surgical approach, our division established laparoscopy as the technique of choice for nephrectomy in children with MCDK [ 17 ]. It is indicated for the dysplastic kidney with size > 5 cm after the first year of life, or < 5 cm with no signs of regression in three subsequent years of clinical follow-up.…”
