Background: Day-surgery, laparoscopic cholecystectomy (DSLC) becomes the standard management of chronic gall bladder diseases worldwide due to its safety and feasibility. The aim of our study was to assess DSLC at two secondary care governmental hospitals in Saudi Arabia with a separate day-surgery unit regarding complications, same-day discharge, to identify problems associated with the early discharge, and determine the degree of patient satisfaction. Methods: Over a 3-years period, all patients undergoing elective laparoscopic cholecystectomy under the authors were prospectively studied. All patients were asked on following up post-operatively about their satisfaction and recommendation of DSLC for their relatives and friends. Results: Eight hundred and sixteen patients underwent elective day surgery laparoscopic cholecystectomies, predominantly female (89.7%) and ASA I (77.2%) with a mean age 38.1 2 18.7 years. Eighty-eight percent of patients were discharged directly from the day surgery unit, and further 1.6% of patients (so nearly 89.6% of the patients) were discharged within 24 hours from the operation. Readmission following hospital discharges was necessary for three patients (0.37%). The overall rate of complications were 1.35% (11patients). At follow-up, 701 patients (97.36%) were satisfied with their experience undergoing DSLC. Conclusions: Our findings confirmed that DSLC is safe and feasible. Many patients showed high satisfaction with their length of stay and day surgery ward care although we extend the criteria for DSLC.

Barely 130 years after its first description, cholecystectomies are among the most commonly performed surgeries in the USA. The success of this operation with subsequent technical improvements, such as laparoscopic approaches, caused a paradigm shift in the management of gallstone disease. However, symptoms persist in 10-40 % of successfully operated patients. Reviewing monographs, textbooks, and articles published during the last 300 years, several important factors emerge as likely contributors to limited or poor treatment responses. Early on, clinicians recognized that cholelithiasis is quite common and thus often an incidental finding, especially if patients present with vague or atypical symptoms. Consistent with these observations, patients with such atypical symptoms are less likely to benefit from cholecystectomy. Similarly, lasting improvements are more reliably seen in patients with symptoms of presumed biliary origin and documented gallstones compared to individuals without stones, an important point in view of increasing rates of surgery for biliary dyskinesia. While cholelithiasis can cause serious complications, the overall incidence of clinically relevant problems is so low that prophylactic cholecystectomy cannot be justified. This conclusion corresponds to epidemiologic data showing that the rise in elective cholecystectomies decreased hospitalizations due to gallstone disease, but was associated with a higher volume of postoperative complications, ultimately resulting in stable combined mortality due to gallstone disease and its treatment. These trends highlight the tremendous gains in managing gallstone disease, while at the same time reminding us that the tightening rather than expanding indications for cholecystectomy may improve outcomes.

Сдержанное отношение хирургов к сокращению сроков пребывания больных в стационаре и внедрению стационаров одного дня обусловлено в том числе недостаточной разработкой клинических критериев и подхо-дов к решению этого вопроса. Цель исследования -разработка программы хирургического лечения больных желчнока-менной болезнью в условиях стационара одного дня. Материал и методы. в статье приводятся результаты хирургического лечения 161 больного желчнокаменной болезнью. всем больным в плановом порядке выполнялась ЛХЭ. 41 больной опери-рован в условиях стационара одного дня (1-я группа), 120 больных -в хирургическом отделении стационара (2-я группа). Результаты. Частота интраоперационных осложнений в 1-й группе была 2,4%, во 2-й группе -3,4% (р>0,05). Частота после операционных осложнений оказалась достоверно выше во 2-й группе (8,3% против 2,4% в 1-й группе при р<0,05). Средняя продолжительность стационарного лечения в 1-й группе составила 1 койко-день, во 2-й группе -4,4 койко-дня. Вывод. Обоснована возможность лечения больных хроническим калькулезным холециститом в стационаре краткосроч-ного пребывания, уточнены показания к этой методике лечения. доказана безопасность такого подхода в лечении этой категории больных при условии тщательного их отбора. разработан алгоритм хирургического лечения больных хрониче-ским калькулезным холециститом в условиях стационара краткосрочного пребывания.
