Laparoscopic Cholecystectomy for Acute Cholecystitis: Prospective Trial

Abstract: This prospective study determines the indications for and the optimal timing of laparoscopic cholecystectomy (LC) following the onset of acute cholecystitis. It also evaluates preoperative and operative factors associated with conversion from laparoscopic cholecystectomy to open cholecystectomy in the presence of acute cholecystitis. Having been established as the procedure of choice for elective cholelithiasis, LC is now also used for management of acute cholecystitis. Under these circumstances the procedure … Show more

“…A cohort study that aimed to compare the results between expert surgeons and surgical trainees found no significant differences in either the conversion rate or the operating time (Elder, 1996). A similar study noted that mortality risk was lower with LC than with the open procedure (Zacks, 2002).…”

“…Four SRs (ANDEM, 1994;Downs, 1996;Korolija, 2004;Keus, 2006a;Keus, 2006b), two RCTs (Mimica, 2000;Ros, 2001), three cohort studies (Devereaux, 2005;Elder, 1996;Finan, 2006), one CPG (Zacks, 2002), one cross-sectional study (Collet, 1997) and one case series (Ibrahim, 2006) were included. One SR concluded that laparoscopic techniques take longer than conventional techniques.…”

“…[1][2][3][4] The safety 3,5,6 and the optimal timing of laparoscopic cholecystectomy [1][2][3]7 have been studied; generally, the outcomes have been good when laparoscopic cholecystectomy is carried out early. [1][2][3][4][5] In addition, several studies have evaluated laparoscopic cholecystectomy after selective sustained percutaneous transhepatic gallbladder drainage (PTGBD) for the control of severe inflammation 3,[8][9][10] and improvement of cholecystitis-induced disorders. Especially, Chikamori et al 11 reported on 16 laparoscopic cholecystectomy patients with PTGBD who experienced a reduced duration of surgery and a lower rate of conversion to open laparotomy compared with laparoscopic cholecystectomy pa-tients who did not receive PTGBD and also those who underwent delayed laparoscopic cholecystectomy after conservative therapy.…”
