“…How surgery allows homeostatic mechanisms to be altered is poorly understood, however, it is likely that gut-brain neuro-endocrine signalling is engaged, and last century's dogmas of "restriction" or "malabsorption" as key putative mechanisms for today's commonly used procedures can be largely dismissed. Since the 1980s studies have shown consistently that gastric only procedures such as gastric stapling, AGB and SG do not delay the transit of food within the foregut and any minor delays are discordant with timing of satiety generated by these procedures (Dixon et al, 2012). In addition there is no evidence of significant macronutrient malabsorption after any of the commonly used procedures, RYGB, AGB and SG (Miras and le Roux, 2013).…”

“…Remarkably, both procedures demonstrated excellent sustained weight loss if the anatomical arrangement could be maintained. Indeed, the need to provide the subtle adjustments to gastric pressure, stretch and tension for individual patients with the vertical banded gastroplasty or a fixed gastric band in order to achieve optimal early satiation and prolonged satiety following a meal led to the simultaneous development of adjustable gastric banding in Sweden and the United States (Dixon et al, 2012). The effect of adjustability to influence satiety has been shown in a double blinded randomised controlled trial with the band either correctly adjusted or empty.…”

“…10 We previously demonstrated that randomized controlled trials comparing surgical weight loss with conventional therapy are feasible, and LAGB is effective, safe, and cost-effective. 11,12 We conducted a 2-year randomized controlled trial involving 60 severely obese patients comparing LAGB with conventional weight loss therapy for the management of moderate to severe OSA.…”
