Laparoscopic Adjustable Gastric Banding After Failed Roux-En-Y Gastric Bypass

Abstract: In this largest published cohort, secondary banding of failed RYGB provides only limited additional weight loss. Furthermore, this technique is associated with high morbidity and reoperation rates. A significant difference in effect was found between patients with weight loss failure and weight regain. Larger prospective series are necessary to evaluate if the modest benefits are worth the risks of secondary LAGB.

“…The nine re-operated patients achieved a weight loss of 12.0 kg (SD = 10.8), recalculated as 35.9% EWL (SD = 36.2) and 29% TWL (SD = 5.2) 24 months after surgery. This result differs from the weight loss that was found earlier in forty re-operated patients lacking an MDT approach (17.6% EWL, SD = 28.3) [9]. This difference suggests that weight loss outcomes of surgically treated non-responders have been improved due to the introduction of an MDT.…”

“…The most interesting nding is that the minority of patients (13%) underwent revisional surgery, while all other patients (87%) were considered as not eligible for surgery. Before the introduction of an MDT evaluation in our center, the rate of revisional surgery was much higher (68% versus 13%) [9]. Based on these ndings, it is likely to assume that an MDT approach reduced the indication for revisional surgery.…”

“…El BGYR está asociado con un fracaso entre el 15 y 35% 5 . La BGA laparoscópica en pacientes con BGYR previo es una técnica segura, que ha demostrado resultados efectivos en el porcentaje de pérdida de exceso de peso (PEP) en series pequeñas de pacientes, hasta un 38% PEP con seguimiento a 1 año 5 .…”
