Laparo-endoscopic versus open recurrent inguinal hernia repair: should we follow the guidelines?

Abstract: IntroductionOn the basis of six meta-analyses, the guidelines of the European Hernia Society (EHS) recommend laparo-endoscopic recurrent repair following previous open inguinal hernia operation and, likewise, open repair following previous laparo-endoscopic operation. So far no data are available on implementation of the guidelines or for comparison of outcomes. Besides, there are no studies for comparison of outcomes for compliance versus non-compliance with the guidelines.Patients and methodsIn total, 4812 p… Show more

“…In a registry study, 90.5% of patients out of 2,482 laparo-endoscopic recurrent repair operations and only 38.5% of patients out of the 2,330 open recurrent repair procedures were operated on in accordance with the guidelines [52]. The authors concluded that for recurrent inguinal hernia repair, the guidelines are not yet being observed to the extent required [52].…”

“…The authors concluded that for recurrent inguinal hernia repair, the guidelines are not yet being observed to the extent required [52]. Paying heed to the guidelines, comparison of perioperative and 1-year outcome for laparo-endoscopic and open repair for primary versus recurrent male unilateral inguinal hernia showed significant differences to the disadvantage of the recurrent operation [53,54].…”

“…While that approach assures the best outcome (4), even when these guidelines are followed a re-recurrence rate of 1–2% is seen on 1-year follow-up, with that rate rising further in the subsequent years (3). Accordingly, increasingly more patients will in the future seek treatment for re-recurrence after anterior and posterior mesh implantation in the groin.…”

“…Recurrences after inguinal hernia repairs can occur even up to 50 years later [2]. In recent administrative data and registry analyses it was revealed that the proportion of recurrent repairs in the total collective of inguinal hernia repairs among men was 11.3-14.3% [3][4][5][6][7] and in women 7.0-7.4% [4,8]. By contrast, in systematic reviews and meta-analyses the recurrence rates were still far lower (1.2-3%) than those cited above since the included studies had a maximum follow-up time of 6 years [9][10][11].…”

“…laparo-endoscopic (TEP, TAPP) following previous open anterior repair and anterior open (Lichtenstein) following previous laparo-endoscopic repair. However, to date that recommendation is not adequately applied [5] and results in significantly higher rates of second recurrences [5].…”
