Langerhanszell-Histiozytose im Kindesalter - Ergebnisse der DAL-HX 83 Studie*

Abstract: Owing to the unclear and mostly unknown etiology of Langerhans' cell Histiocytosis (LCH) and the unsatisfactory results in treating disseminated LCH a prospective multicentric study DAL-HX 83 was commenced, including 45 different clinics of West-Germany, Austria and Netherlands. From June 1st, 1983 to October 31st, 1986, 97 patients (pts) were involved in this study. 35 pts (9 females, 26 males, medium age 6 2/12 years, age range 0/12-14 2/12 years) suffering from localized disease (28x unifocal bone, 6x isola… Show more

“…The rate of diabetes insipidus in our group (20%) is lower than the 35% in Sims' study or the 36% reported by McLelland et al but is higher than the 4% of the DAL-HX'83 study [25]. This may reflect a referral bias in our patients, or, alternatively, it may be due to a prompt systemic chemotherapeutic approach and a shortened course of active disease.…”

“…Overall morbidity in our series was 47.7%, lower than that reported in other studies, such as those of Lahey [21], Sims [22] and McLelland et al [23], and similar to that of Komp et al [24]. “…The observation that the prognosis for children with histiocytosis X is related to the extent of organ involvement and dysfunction (LAHEY 1975) has been confirmed in several large retrospective reviews of LCH BERRY et al 1986;GADNER et al 1987;RANEY and D'ANGIO 1989;McLELLAND et al 1990). The Lavin-Osband modification of the Lahey staging system is outlined in Table 22.1.…”

“…Data on 223 patients gathered from four review articles4 5 6 7 suggest that Langerhans' cell histiocytosis might also develop as a consequence of two rate limiting events (figure). x 2 Analysis shows a significant difference in the age at diagnosis between the 75 patients with single bone lesions and the 148 patients with multiorgan disease (P<0.001).…”
