Lamotrigine

Abstract: Lamotrigine has been shown to be an effective maintenance therapy for patients with bipolar I disorder, significantly delaying time to intervention for any mood episode. Additionally, lamotrigine significantly delayed time to intervention for a depressive episode and showed limited efficacy in delaying time to intervention for a manic/hypomanic episode, compared with placebo. Although not approved for the short-term treatment of mood episodes, lamotrigine has shown efficacy in the acute treatment of patients w… Show more

“…Antidepressant efficacy has not been demonstrated for standard antidepressants or typical antipsychotics, either as acute treatments for bipolar depression, or as maintenance therapy for the prevention of depression relapse 4, 5, 6, 7, 8. There is weak evidence for the maintenance efficacy of most mood stabilizers in the prevention of depressive relapse (notably lithium, valproate, carbamazepine),9 and stronger evidence suggesting that lamotrigine may be effective in delaying depressive episode recurrence 10…”

“…14 At present there is no evidence that lamotrigine destabilises mood when used to treat acute mood episodes or as maintenance, so it can be introduced from the beginning of a depressive episode.…”

“…While the half-life of lamotrigine is around 22 hours in drug naïve patients, indicating that once-a-day dosing would be feasible, the metabolism may be induced or inhibited [16]. The half-life of lamotrigine in patients taking enzyme inducers is around 12 hours [21]. The clearance of lamotrigine is significantly reduced in patients taking valproic acid, due to inhibition of lamotrigine’s glucuronidation by valproate [51].…”
