Background. Recurrent perianal inflammation has great etiologic diversity. A possible cause is cow's milk allergy (CMA). The aim was to assess the magnitude of this cause. Subjects and Methods. This follow up clinical study was carried out on 63 infants with perianal dermatitis of more than 3 weeks with history of recurrence. Definitive diagnosis was made for each infant through medical history taking, clinical examination and investigations including stool analysis and culture, stool pH and reducing substances, perianal swab for different cultures and staining for Candida albicans. Complete blood count and quantitative determination of cow's milk-specific serum IgE concentration were done for all patients. CMA was confirmed through an open withdrawal-rechallenge procedure. Serum immunoglobulins and CD markers as well as gastrointestinal endoscopies were done for some patients. Results. Causes of perianal dermatitis included CMA (47.6%), bacterial dermatitis (17.46%), moniliasis (15.87%), enterobiasis (9.52%) and lactose intolerance (9.5%). Predictors of CMA included presence of bloody and/or mucoid stool, other atopic manifestations, anal fissures, or recurrent vomiting. Conclusion. We can conclude that cow's milk allergy is a common cause of recurrent perianal dermatitis. Mucoid or bloody stool, anal fissures or ulcers, vomiting and atopic manifestations can predict this etiology.

La diarrhée fonctionnelle est la première cause de diarrhées hydroélectrolytiques. Elle regroupe le syndrome de l'intestin irritable à forme diarrhéique (SII-D) et la diarrhée fonctionnelle indolore. Elle correspond à une situation clinique au cours de laquelle les patients décrivent l'émission de selles plus ou moins nombreuses, pâteuses ou liquides, de manière continue ou récidivante, avec un bilan paraclinique excluant l'existence d'une lésion muqueuse digestive, une colite microscopique et une hypersécrétion hormonale. La frontière avec une forme minime de maladie coeliaque n'est pas toujours aisée à déterminer. D'un point de vue physiopathologique, la diarrhée peut s'expliquer par des troubles de la motricité intestinale, une hypersensibilité viscérale, une malabsorption des acides biliaires, une pullulation bactérienne endoluminale qui peut, elle-même, favoriser une malabsorption de certains sucres tels que le fructose qui aggrave la diarrhée. D'un point de vue thérapeutique, le lopéramide et la colestyramine sont les principaux traitements actuels de la diarrhée. Ces médicaments sont associés aux antispasmodiques, au citrate d'alvérine, aux argiles ou aux antidépresseurs pour soulager la douleur en cas de SII-D. Les probiotiques paraissent une solution intéressante. La place des antibiotiques est à déterminer.Les cinq points utiles à la thérapeutique • L'élimination initiale d'une fausse diarrhée de constipation est essentielle dans la stratégie diagnostique et thérapeutique ;• le lopéramide et la colestyramine sont les principaux traitements actuels de la diarrhée, que celle-ci soit douloureuse ou non ;• ces médicaments sont associés aux antispasmodiques, au citrate d'alvérine, aux argiles ou aux antidépresseurs pour soulager la douleur en cas de syndrome de l'intestin irritable à forme diarrhéique ;• en cas d'échec, la prégabaline ou la gabapentine sont d'éventuelles options ;• les probiotiques paraissent une solution intéressante, en cours d'évaluation.
