Lactobacillus GG Administered in Oral Rehydration Solution to Children with Acute Diarrhea: A Multicenter European Trial

Abstract: Administering oral rehydration solution containing Lactobacillus GG to children with acute diarrhea is safe and results in shorter duration of diarrhea, less chance of a protracted course, and faster discharge from the hospital.

“…In RCTs investigating their therapeutic efficacy, they have been generally used in association with ORS (293). In RTCs, L. rhamnosus GG alone or in combination with ORS has demonstrated therapeutic efficacy in significantly reducing by at least half the duration of acute diarrhea mainly resulting from rotavirus infection in children (296)(297)(298)(299)(300)(301)(302)(303)(304)(305)(306)(307)(308)(309)(310). Two meta-analyses have been conducted to specifically examine the RCTs conducted with L. rhamnosus GG (311,312).…”

“…While the numerous experimental in vitro and in vivo data reported above demonstrated the bactericidal activity of L. rhamnosus GG against enterovirulent bacteria, it is both surprising and intriguing to note that therapeutic efficacy was absent in RCTs conducted with this probiotic strain in children with established bacterially induced diarrhea (302,305,314,326). Consistent with the in vitro and in vivo antibacterial activities reported above for live and heat-treated L. acidophilus strain LB against enteroadherent and enteroinvasive bacteria (122,137,147,181,182), two RCTs have demonstrated the therapeutic efficacy of Lacteol in combination with ORS in decreasing the frequency and duration (reduced by at least half) of enterovirulent bacterium-associated acute watery diarrhea in children between the ages of 1 and 4 years (207,327).…”

“…The strongest evidence of a beneficial effect of probiotics has been established with L. rhamnosus GG and B. lactis BB-12 for prevention and L. reuteri SD2222 for treatment (51,52,62,80,122,131,138) of acute diarrhea mainly caused by rotaviruses in children ( Table 1). The study designs cited are similar, randomized, double blinded, and placebo controlled.…”

“…Note also that unlike many pharmaceutical treatments, there were no significant side effects reported. In a European study, faster hospital discharge was achieved in addition to improvement in clinical outcome (51). Two hundred ninety-one children 1 to 3 months of age were randomly allocated to receive oral rehydration solution plus placebo or 10 10 L. rhamnosus GG.…”

“…Among many validated useful characteristics, research most commonly acknowledges positive aspects in the treatment of acute and chronic diarrhea in children and adults (Isolauri et al, 1991;Guandalini et al, 2000;Gupta et al, 2000;Saavedra, 2000).…”
