Lack of Response to Isotretinoin in Annular Elastolytic Giant Cell Granuloma

Abstract: Annular elastolytic giant cell granuloma is a rare disease characterized by annular configurated lesions with typical histopathologic findings. We report a case of annular elastolytic giant cell granuloma in a 50-year-old male patient. Treatment modalities for this disease are limited and mostly unsatisfactory, although isotretinoin was found to be effective in a single case. However, systemic isotretinoin treatment of 12 weeks' duration was of no benefit in the presented case.

“…[2,3,[9][10][11][12][13][14] In contrast, topical corticosteroids, PUVA, cauterization, cryotherapy, methotrexate, and isotretinoin were found to be unsuccessful. [2,15] In addition to that, clofazimine has been reported as both effective [16] and ineffective [2] in two separate cases.…”

“…The pathogenesis of AEGCG remains unclear; but it is thought that ultraviolet radiation, heat or other factors might change the antigenicity of elastic fibres and induce a cellular immune reaction 1 . In addition, there have been reports of AEGCG associated with systemic disorders 2–5 . We report a case of AEGCG in a patient with newly diagnosed diabetes mellitus (DM), and review the cases of AEGCG in the Japanese literature.…”

“…Thus, DM may be a precipitating factor for AEGCG, possibly contributing to structural damage of the elastic fibres. Only a few cases of AEGCG associated with DM have been reported in the English literature 4,5 . Further studies are needed to clarify the relationship between AEGCG and DM.…”
