Lack of effect of grapefruit juice on the pharmacokinetics and pharmacodynamics of amlodipine

Abstract: Aims To determine whether repeated once daily administration of grapefruit juice altered the pharmacokinetics or pharmacodynamics of the calcium antagonist amlodipine. Methods The effects of grapefruit juice on the pharmacokinetics and pharmacodynamics of oral and intravenous amlodipine were assessed in 20 healthy men in a placebo‐controlled, open, randomized, four‐way crossover study using single doses of amlodipine 10 mg. For 9 days beginning with the day of administration of amlodipine, grapefruit juice… Show more

“…A milder but significant interaction has been detected with nitrendipine, pranidipine and nimodipine that exhibited bioavailability increments of 100, 73 and 50%, respectively (Soons et al, 1991;Fuhr et al, 1998;Hashimoto et al, 1998). However, little or no effect has been detected for nifedipine and amlodipine (Rashid et al, 1993;Josefsson et al, 1996;Vincent et al, 2000). These two dihydropyridines are characterized by a high bioavailability compared to felodipine, nisoldipine and nicardipine, which can explain the lack of the interaction.…”

“…However, the pharmacokinetics of many other drugs was not altered by grapefruit juice. For example, grapefruit juice did not alter the bioavailability of digoxin, diltiazem, and amlodipine in human volunteers, and indinavir in HIV-positive patients (Becquemont et al, 2001;Sigusch et al, 1994;Vincent et al, 2000). Although these drugs undergo presystemic metabolism, CYP3A4 just contributes to a minor extent.…”

“…Drugs that have their serum concentrations increased by coadministration with grapefruit juice are listed in Table 7. [126][127][128][129][130][131][132] Patients should be advised to avoid taking medications on this list with grapefruit, unless advised to do so by their physician under controlled conditions.…”
