Purpose To investigate the changes in choroidal thickness (CT), axial length (AL), and ocular perfusion pressure (OPP) accompanying intraocular pressure (IOP) reduction after trabeculectomy. Methods Thirty-nine eyes of 39 patients with primary open-angle glaucoma uncontrolled by medical therapy were included in this prospective and interventional study. All patients underwent a fornix-based trabeculectomy. The CT was measured by enhanced depth imaging-optical coherence tomography. IOP, AL, and systolic/ diastolic blood pressure were also measured, and OPP was calculated. All measurements were performed at baseline and 1 month after surgery. Results The mean IOP was 25.0 ± 5.8 mm Hg at baseline and 11.7 ± 2.6 mm Hg after trabeculectomy (Po0.001), and the mean subfoveal CT was 295±84 mm Hg at baseline and 331±82 mm Hg after trabeculectomy (Po0.001). The mean AL was 23.64 ± 0.98 mm at baseline and 23.54 ± 0.96 mm after trabeculectomy (Po0.001), whereas the mean OPP was 38.8 ± 6.2 mm Hg preoperatively, and 51.1 ± 7.3 mm Hg postoperatively (Po0.001). The change in CT negatively correlated with the change in IOP (r ¼ À 0.785, Po0.001) and AL (r ¼ À 0.693, Po0.001), whereas it positively correlated with the change in OPP (r ¼ 0.418, P ¼ 0.008). Conclusion These results suggest that the large IOP decrease following trabeculectomy causes choroidal thickening.In addition, CT changes are associated with IOP and AL reduction as well as OPP increase.

PURPOSE. To study change in choroidal thickness (CT) after water drinking test (WDT), comparing angle closure (AC) to open angle (OA) eyes.METHODS. Before and 30 minutes after drinking 1 L of water, 88 glaucoma subjects underwent measurements of CT by spectral domain-optical coherence tomography, IOP, blood pressure (BP), axial length (AL), and anterior chamber depth (ACD). RESULTS.Baseline CT was significantly greater in AC than in OA eyes (P ¼ 0.002). After WDT, BP, IOP, and AL increased significantly (all P 0.0001). Mean CT increased significantly in the AC group (5.6 lm, P ¼ 0.04, n ¼ 40) and among 80 subjects whose IOP rose > 2 mm Hg (responders; 3.2 lm, P ¼ 0.048), but not in the OA group or among all subjects (2.5 lm increase overall, <1% of baseline CT, P ¼ 0.10). ACD decreased in AC (À18 lm, P ¼ 0.07), but not in OA eyes (þ3 lm, P ¼ 0.74). AC eyes had a significantly greater IOP increase after WDT than OA eyes (P ¼ 0.002, multivariate regression). Among responders, CT increased more in those with larger diastolic perfusion pressure increase and in AC compared to OA eyes (P ¼ 0.04 and P ¼ 0.053, respectively, multivariate regression).CONCLUSIONS. A significant increase in CT and a decrease in ACD after WDT were observed in AC but not in OA eyes, and IOP increased significantly more in AC than in OA eyes, suggesting that the dynamic behavior of the choroid may play a role in the AC process. Purpose: To evaluate variations in choroidal thickness (CT) during the water drinking test (WDT) in emmetropic eyes (EE) and highly myopic eyes (ME) using spectral-domain optical coherence tomography (SD-OCT). Methods: Clinical trial performed at a tertiary care hospital comprising 30 randomly selected eyes. The WDT and SD-OCT macular scans were performed 10 and 45 min after water ingestion in 15 myopic and 15 EE of 15 healthy patients in each group. Primary study outcomes were average macular CT measured by SD-OCT and intraocular pressure (IOP) during the WDT. Results: The mean spherical equivalent refraction was 0.15 ± 0.24 D in emmetropic and -7.1 ± 1.75 D in ME (p<0.001). No statistical differences between EE and ME were observed during the WDT response. EE had higher CT compared with ME at the fovea (361.4 ± 55.4 vs 257.9 ± 95.3; p<0.001), 3 mm nasal to the fovea (158.0 ± 71.8 vs 122.5 ± 54.5; p=0.047), and 3 mm temporally to the fovea (310.6 ± 52.4 vs 247.6 ± 90.1; p=0.05). Regarding CT variation, significant differences in foveal CT at 10 min after water ingestion were observed in both EE and ME, with no statistically significant difference observed between groups. A moderate correlation between IOP peak during the WDT and CT was demonstrated in ME (r=0.52; p=0.04). Conclusions:No statistically significant differences in CT variation during the WDT were observed between EE and ME, indicating similar behavior of the choroidal bed during the WDT in both groups. Further, CT was thinner in highly ME, with CT variation unable to explain elevations in IOP observed during the WDT.
