Background: The considerable increase in labiaplasty requires plastic surgeons to be acquainted with the surgical techniques, their respective complications, and satisfaction rates. The authors conducted a systematic review of the available evidence on labia minoraplasty, looking at surgical techniques, clinical outcomes, and patient satisfaction. Methods: A comprehensive search across PubMed, Web of Science, SCOPUS, and Cochrane CENTRAL was performed through October of 2020. A random-effects model meta-analysis was performed to assess satisfaction and complication rates. Results: Forty-six studies including 3804 patients fulfilled the inclusion criteria. The pooled satisfaction rate after labia minoraplasty was 99 percent (95 percent CI, 97 to 99 percent). Substantial heterogeneity was present across studies (I2 = 63.09 percent; p < 0.001). The highest pooled incidence of dehiscence was reported for laser-assisted labiaplasty (5 percent; 95 percent CI, 2 to 8 percent) and wedge resection (3 percent; 95 percent CI, 1 to 5 perecent). The highest pooled incidence of hematoma formation (8 percent; 95 percent CI, <1 to 23 percent) and postoperative bleeding (2 percent; 95 percent CI, <1 to 15 percent) was reported for W-shape resection. The highest pooled incidence of transient pain or discomfort was reported for de-epithelialization (2 percent; 95 percent CI, <1 to 23 percent) and W-shape resection (2 percent; 95 percent CI, <1 to 15 percent). Three cases of flap necrosis were reported; two occurred using wedge resection labia minoraplasty and one was not specified. The most common causes for revision surgery (n = 169) were dehiscence and aesthetic concerns. Conclusions: Labia minoraplasty is a safe procedure, but serious complications requiring surgical management have been reported. Surgical experience, knowledge of female genital anatomy, and thorough technique selection in accordance with the patient characteristics are mandatory.

Introduction and hypothesis:The current terminology used to describe cosmetic gynecologic procedures includes many nondescriptive, trademarked, or informal names, which contributes to substantial ambiguity about their aims and specific techniques. The development of clear, uniform descriptive terminology for cosmetic gynecology is needed for patients, researchers, and practitioners across multiple specialties.Methods: This document was developed from a collaboration of selected members from the International Urogynecological Association (IUGA) and the American Urogynecologic Society (AUGS). Wide-ranging literature reviews were performed to identify the breadth of currently used terms and tools for measuring efficacy and safety. After extensive internal review the adoption of each definition was ratified by group consensus.Results: A terminology report for elective cosmetic gynecology procedures, anatomical classification, outcome metrics, and reporting of complications has been developed. This document seeks to provide clear descriptive guidance for patients, researchers, and practitioners across multiple specialties. This document will be subject to internal review by IUGA and AUGS to incorporate and adopt evidence-based changes in the field.Conclusions: A consensus-based document establishing clear terminology for cosmetic gynecology procedures has been created. Use of these terms should be encouraged to provide clarity to patients seeking these procedures and to facilitate future research to establish the safety and efficacy of these procedures.

Cosmetogynecological Surgery With Local Anesthesia: Preliminary Experience

Introduction: Although cosmetic gynecological procedures have been reported since antiquity, recent expansion made the cosmetogynecology becomes a recognized subspecialty. Presently, minimally invasive surgeries allow some of them to be performed under local anesthesia (LA). We present our preliminary experience using LA, highlighting safety and advantages. Methods: A total of 246 surgeries under local anesthesia on 185 median age patients, 39 years old (range: 14-80) between July 2020 and July 2021 in a Chilean aesthetic gynecology center, were retrospectively reviewed. Results: The most common surgery was Labiaplasty Type I, covering 40% (n = 99). Twenty percent of all patients (n = 38) underwent multiple surgeries in one surgical act. Minor complications were observed in 8% (n = 20) of surgeries, with satisfactory resolutions, without complications inherent to the use of LA or vasoconstrictors. Patients reported high degree of satisfaction, even in cases followed by labiaplasty repair or minor complication. Labiaplasty-related costs were considerably reduced compared with usual anesthetic procedure in more complex centers. Conclusion: This is the first South American report of various aesthetic genital procedures carried out under local anesthesia. In our experience, local anesthesia provided safe and effective results.
