Background Cerebral venous thrombosis (CVT) is a rare cerebral vascular disease, the presentation of which is highly variable clinically and radiologically. A recent study demonstrated that isolated subarachnoid hemorrhage (iSAH) in CVT is not as rare as thought previously and may have a good prognostic significance. Hemorrhagic venous infarction, however, is an indicator of an unfavorable outcome. We therefore hypothesized that patients who initially suffered iSAH would have a better clinical outcome than those who suffered hemorrhagic cerebral infarction. Patients and methods We selected patients hospitalized due to CVT, who presented either with isolated SAH or cerebral hemorrhagic infarction at admission or during the following 24 hours: 23 (10 men) aged 22–73 years. The data were extracted from hospital admission records, our computer data system, and the hospital radiological database. Results The iSAH group consisted of 8 (6 men) aged 49.3 ± 16.2 and the hemorrhagic infarction group included 15 (4 men) aged 47.9 ± 16.8. Despite having a significantly greater number of thrombosed venous sinuses/deep veins (Mann-Whitney Rank Sum Test, p = 0.002), the isolated SAH group had a significantly better outcome on its modified Rankin Score (mRs) than the hemorrhagic infarction group (Mann-Whitney Rank Sum Test, p = 0.026). Additional variables of significant impact were edema formation (p = 0.004) and sulcal obliteration (p = 0.014). Conclusions The patients who suffer iSAH initially had a significantly better outcome prognosis than the hemorrhagic infarction patients, despite the greater number of thrombosed sinuses/veins in the iSAH group. A possible explanation might include patent superficial cerebral communicating veins.

A New Classification of the Anatomical Variations of Labbé’s Inferior Anastomotic Vein

(1) Background: The inferior anastomotic vein of Labbé (LV) courses on the temporal lobe, from the sylvian fissure towards the tentorium cerebelli and finishes at the transverse sinus (TS). The importance of the LV topography is related to skull base neurosurgical approaches. Based on the hypothesis of the existence of as yet unidentified anatomical possibilities of the LV, we aimed through this research to document the superficial venous topographic patterns at the lateral and inferior surfaces of the temporal lobe. (2) Methods: A retrospective cohort of 50 computed tomography angiograms (CTAs) of 32 males and 18 females was documented. (3) Results: Absent (type 0) LVs were found in 6% of cases. Anterior (temporal, squamosal–petrosal–mastoid, type 1) LVs were found in 12% of cases. LVs with a posterior, temporoparietal course (type 2) were found to be bilateral in 46% of cases and unilateral in 36% of cases. Type 3 LVs (posterior, parietooccipital) were found to be bilateral in 8% and unilateral in 32% of cases. In 24% of cases, duplicate LVs were found that were either complete or incomplete. A quadruplicate LV was found in a male case. On 78 sides, the LV drained either into a tentorial sinus or into the TS. (4) Conclusions: The anatomy of the vein of Labbé is variable in terms of its course, the number of veins and the modality of drainage; thus, it should determine personalized neurosurgical and interventional approaches. A new classification of the anatomical variations of Labbé’s vein, as detected on the CTAs, is proposed here (types 0–3).

Цель исследования -изучить хирургическую анатомию вены Лаббе (нижней анастомотической вены) и вен, дренирующих медиобазальную поверхность височной доли, а также выделить те наиболее часто встречающиеся варианты венозного дренажа латеральных и базальных отделов височной доли, которые необходимо учесть при осуществлении латеральных нейрохирургических доступов. Материалы и методы. Исследование проведено на 50 нефиксированных гемисферах головного мозга с гомолатеральной поверхностью основания черепа. Хирургическая часть работы выполнена на 25 трупах. Моделировали передний и задний петрозальные подвисочные транстенториальные доступы. Использовали стандартный нейрохирургический набор микроинструментов, операционный микроскоп и высокоскоростные нейрохирургические дрели. На интрадуральном этапе доступа идентифицировали вену Лаббе, определяли ее анастомотический тип и характер сброса в венозные коллекторы основания черепа. Результаты. Идентифицировано 115 групп дренирующих височных вен. Дренирующие вены трансверсальной группы обнаружены в 100 % случаев, тенториальной группы -в 72 %, вены петрозальной группы -в 58 %. Не выявлено существенных различий между местами впадения вен справа и слева. Выделены 3 варианта венозного дренажа латеральных и базальных отделов височной доли: 1) впадение групп дренирующих вен в синодуральный угол, заднюю треть каменистого синуса и передние отделы поперечного синуса -14 (28 %) случаев; 2) впадение в синодуральный угол, передние отделы поперечного синуса и поверхность намета мозжечка -21 (42 %) случай; 3) впадение во все венозные коллекторы (синодуральный угол, каменистый, поперечный и тенториальные синусы) -15 (30 %) случаев. Выявлены 2 анастомотических типа вены Лаббе: с прямым (12 %) и непрямым (88 %) анастомозом с венозными коллекторами основания черепа. При непрямом типе наблюдалась «венозная звезда», состоящая из нескольких венозных стволов, которые сходились в области синодурального угла. При непрямом типе анастомоза вена Лаббе впадала в каменистый синус (4 % случаев), синодуральный угол (14 %), тенториальные синусы (10 %) и поперечный синус (60 %) через переднюю нижнюю поверхностную височную вену (8 %), среднюю нижнюю поверхностную височную вену (26 %) и заднюю нижнюю поверхностную височную вену (54 %). Вены Лаббе с прямым типом анастомоза впадали только в поперечный синус. В большинстве случаев вена Лаббе впадала в поперечный синус более чем на 10 мм кзади от синодурального угла. Вены крупного диаметра (доминантные вены) были обнаружены в 38 % наблюдений, все они впадали в поперечный синус. Заключение. Выделенные нами варианты венозного дренажа латеральных и базальных отделов височной доли и анастомотические варианты вены Лаббе дают возможность выбрать оптимальный нейрохирургический доступ и скорректировать его направление, уточнить характер тракции височной доли и спрогнозировать возникновение технических трудностей при осуществлении подвисочного доступа.
