This article examines the role of social workers involved in a pilot home care project undertaken to attend to the social needs of people at the end of their lives and their carers. Mixed methods approach and evidence-based, constructivist, and comprehensive evaluation were chosen to achieve this aim. Analyses are based on 235 responses from a survey addressed to professionals working in the project, 22 in-depth interviews with a selected group of these professionals, and 114 semi-structured interviews with the sick people and their caregivers. The analysis of the data has considered the theoretical development and professional experience systematized in the Spanish and international literature on social work in end-of-life care. The results highlight the relevance that professionals, patients, and carers attribute to social care at the end of life and to the role, skills, and qualities of social workers. Their contribution has been valued in terms of the management of services and supports (speed, availability, kindness, sensitivity), the establishment of teamwork (coordination, facilitation), and the recognition of the basic principles of the care process (individualization, respect for privacy, confidentiality). Findings demonstrate that social work is a profession well placed to help in the recognition of people's dignity at the end of life, in the exercise of their self-determination, and in the humanization of the care process. Some challenges facing consolidation of this professional practice are also indicated within a framework constrained by a predominantly biomedical culture.

The approval of the Spanish Law for the Regulation of Euthanasia in March 2021 entails a profound social change that has direct implications for professional practice at all levels of care. There is no information available about the experiences of the professionals participating in the process of implementing the law in our country, nor any guide as to what might represent good practices in euthanasia in the context of the Mediterranean countries. Due to the complexity of the concepts often associated with assisted dying processes (such as suffering, dignity and moral compass), it is essential that systematic and detailed research be conducted on how professionals understand their experiences of euthanasia, and that it be conducted during the initial stages. We intend to carry out a qualitative study with a constructionist orientation to gain insight to the social constructs underlying professionals’ initial experiences. It consists of a series of in-depth interviews conducted in two phases: the first being exploratory and the second phenomenological. Sampling will be purposive and substantiated and have the following variation criteria: profession, age, and gender. Participants will be recruited through the Guarantee and Review Commission of Catalonia, and the scope of the study is all of Catalonia. We expect the participation of 31 professionals. The research will be conducted by a multidisciplinary team with the direct participation of researchers from different levels of healthcare, university research centers and civil society through all phases of the study. The expected results are: (a) A detailed description of the experiences of the professionals involved in the implementation of the law; and (b) The identification of the foundations for a guide to good practices in euthanasia in the Mediterranean setting.

Muchos enfermos, al final de su vida, padecen un sufrimiento intenso en el contexto de síntomas refractarios (aquellos que no pueden ser adecuadamente controlados con los tratamientos disponibles, aplicados por médicos expertos). La sedación paliativa es la disminución deliberada del nivel de consciencia del enfermo, con el objetivo de evitar un sufrimiento insoportable causado por uno o más síntomas refractarios. Debe cumplir una serie de requisitos para ser considerada una práctica ética y lícita, como son: la intención de aliviar el sufrimiento, una valoración correcta del sufrimiento y síntomas que lo provocan, la obtención del consentimiento informado y el uso de los fármacos indicados y a las dosis adecuadas. Puede realizarse bien en ámbito hospitalario, bien en domicilio, según preferencias del enfermo, siendo en este último caso la vía subcutánea la de elección. El fármaco de primera línea es el midazolam, tanto intravenoso como subcutáneo, y en el caso de que el síntoma principal sea el delirium utilizaremos la levomepromacina. La sedación puede plantearse de forma intermitente o continua. Es imprescindible una monitorización del enfermo sedado, tanto del nivel de sedación adquirido como de signos o síntomas de sufrimiento, y un acompañamiento de la familia.
