Abstract. A leptospirosis surveillance program was carried out for 12 months on the entire population of the Seychelles. Diagnosis was assessed by both microagglutination test and polymerase chain reaction (PCR) assay. In this population of 74,331, leptospirosis was clinically suspected in 125 subjects and confirmed in 75 patients (incidence of 101 per 100,000; 95% confidence interval ϭ 79-126). Leptospirosis was more frequent in middle-aged males with environmental exposure. Eight serogroups were identified and Icterohaemorrhagiae (31%) and Hurstbridge (20%) were the most frequent. Hurstbridge, a recently identified new serogroup, was implicated in severe cases and death. Influenza-like forms accounted for 37% of the cases while jaundice, acute renal failure, and pulmonary hemorrhage occurred in 52%, 28%, and 19%, respectively. Death occurred in six patients and was related to pulmonary hemorrhage. The PCR result was positive after completion of treatment in eight patients, suggesting that the administered five-day course of penicillin may be inadequate to eradicate the bacteria.

“…Une étude menée entre 1986 et 1990 par l'Institut Pasteur de la Guadeloupe a montré une forte séro-prévalence de cette bactériose au sein des cheptels insulaires de bovins (38 à 90 %), et de chevaux (46 à 70 %) et fait état du décès de six personnes pour la seule année 1993 (Goursaud & Pérez, 1993). Strobel et al (1992) font état d'un taux de prévalence de la leptospirose au sein des populations humaines de la Guadeloupe et de la Martinique 40 fois supérieur à celui de la France métropolitaine, taux comparable à celui établi -285 -pour La Réunion (Duval et al, 1988) mais nettement inférieur à celui de la Nouvelle-Calédonie (200 fois celui de la France métropolitaine, Perrocheau & Pérolat, 1997). Michel (2001) observe, à l'occasion de prélèvements réalisés à l'échelle de peuplements de mammifères d'agro-écosystèmes et d'écosystèmes « naturels », le portage rénal de la bactérie par 17, 37 et 57 % d'un échantillon de Surmulots, Rats noirs et Souris domestiques.…”

The Muridae rodent community of Guadeloupe and Martinique (French West Indies) includes 3 alien species: the Ship Rat (Rattus rattus), the Norwegian Rat (Rattus norvegicus) and the House Mouse (Mus musculus). A 26,740 trap-night effort conducted under standardized sampling methods showed a strong heterogeneity of the species frequency distribution among 4 agricultural ecosystems (sugar cane, banana, cassava and watermelon) and 3 "natural" ones (tropical rain forest and islands or islets covered either totally or partially by a dry vegetation). A second step of the analysis conducted at a smaller space scale, from the surrounding ecosystems of the cultivated areas to the inside of these ones, gave the same result. The House Mouse constituted the majority of the captures in the agricultural ecosystems (56% for the total and 84% for sugar cane specifically). This species is well represented inside the cultivated area and poorly outside. The Ship Rat is well represented in the "savannah" and "thorn-scrub", and in tropical rain forest, habitats that surround sugar cane and banana fields respectively. Statistically less numerous than the others, the Norwegian Rat was nevertheless well represented in the cultivated area devoted to watermelon and cassava, and concentrated in the field margins. The build-up and the optimisation of strategies for controlling the alien rodents in tropical islands are discussed in relation to these results. The build-up of sampling strategies to evaluate epidemiological and environmental risks due to these alien rodents is also discussed. This last discussion was done in the light of a new result showing that 57% of sampled Guadeloupe House Mice hosted Leptospira interrogans on their kidneys.
