We aimed to evaluate the short-term effects of community-based occupational therapy on health-related quality of life and engagement in meaningful activities among women with breast cancer. An open label randomized controlled trial study design was applied. The participants were members of various societies of women with cancer. In total, 22 women have participated in the study. Participants of the experimental group (n = 11) participated in a 6-week community-based occupational therapy program and the usual activities of various societies, whereas the control group (n = 11) women participated in the usual activities of the societies only. 1 of the participants withdrew during the course; therefore 21 completed the study successfully. Participants of both groups were assessed for health-related quality of life and the participants of the experimental group were assessed for engagement in meaningful activities. The evaluation was carried out during the nonacute period of the disease—at the beginning of the study and after 6 weeks. Women of the experimental group demonstrated statistically significantly better scores in the global quality of life, role functions, physical, emotional, cognitive, and social functions, fatigue, insomnia, financial impact, systemic therapy side effects, and breast symptoms scales compared to the control group participants (p < 0.05) after the 6 weeks, as measured by the EORTC QLQ-C30 questionnaire and its breast cancer module QLQ-BR23. Furthermore, women of the experimental group demonstrated significant greater engagement in meaningful activities when applying community-based occupational therapy (p < 0.05), as measured by using the Engagement in Meaningful Activities Survey (EMAS). The evaluation of the associations between the women's engagement in meaningful activities and changes in health-related quality of life showed that greater engagement in meaningful activities was associated with better emotional functions and a lower level of insomnia (p < 0.05). Based on the results of our study, we recommend applying occupational therapy in the field of community healthcare in order to maintain or improve breast cancer patients' health-related quality of life and suggest involving women into meaningful activities during community-based occupational therapy after clarifying which activities are important to them.

Krūties vėžio diagnozę sužinojusios moterys patiria stiprų sukrėtimą, kuris gali paveikti daugelį gyvenimo sričių ir stipriai sutrikdyti pacienčių adaptaciją. Krūties vėžiu sergančių pacienčių potrauminio streso sutrikimo rizika yra didesnė, nei bendrojoje populiacijoje. Tyrimo tikslas – įvertinti sociodemografinių ir klinikinių veiksnių įtaką potrauminio streso sutrikimo rizikai prieš krūties operaciją ir praėjus vieneriems metams po jos. Tyrimo rezultatai parodė, kad krūties vėžiu sergančių pacienčių potrauminio streso sutrikimo rizika per metus statistiškai reikšmingai sumažėjo. Potrauminio streso sutrikimo riziką praėjus vieneriems metams po operacijos didina gyvenimas santuokoje arba partnerystėje ir aukštesnis išsilavinimas. Apmokamas darbas kelia potrauminio streso sutrikimo riziką prieš operaciją, bet apsaugo nuo šios rizikos praėjus vieneriems metams po operacijos. Trumpas laiko tarpas po naviko nustatymo didina potrauminio streso sutrikimo riziką. Krūtį tausojanti operacija, praėjus vieneriems metams po jos, kelia potrauminio streso sutrikimo riziką. Teikiant sveikatos priežiūros paslaugas krūties vėžiu sergančioms pacientėms, tikslinga atkreipti dėmesį į jų psichologines problemas ir poreikius ankstyvajame (prieš operaciją) ir vėlyvajame (praėjus vieneriems metams) sveikatos priežiūros paslaugų teikimo etape. Svarbu į pacienčių sveikatos priežiūros procesą kryptingai įtraukti jų šeimos narius ir artimuosius, taip siekiant sustiprinti pagalbą bei paramą iš artimiausios pacientei socialinės aplinkos.

Factors Affecting Suicidal Thoughts in Breast Cancer Patients

Background and Objectives: Women diagnosed with breast cancer experience severe trauma. Psychological help for breast cancer patients is not sufficient because of limited professional resources. The goal of this study was to identify groups of breast cancer patients with the greatest suicidal risk, who could be the first target for psychosocial interventions. Materials and Methods: The study included 421 women with stage T1–T3/N0–N3/M0 breast cancer. We provided women with a set of questionnaires 1–2 days prior to breast surgery and one year after surgery. One hundred eighty-eight patients completed the questionnaires after one year. We used the Beck Depression Inventory Second Edition (BDI-II) item Suicidal Thoughts or Intentions for the assessment of suicidal risk. The Impact of Event Scale-Revised (IES-R) was used to measure the risk of PTSD and the Vrana–Lauterbach Traumatic Events Questionnaire-Civilian, TEQ-C (TEQ-CV) was used to measure whether patients had experienced other traumatic events in their lifetime. Results: The incidence of suicidal ideation one year after surgery increased from 4.3% to 12.8% of patients. Patients who lived in rural areas had a two times greater risk of suicidal thoughts than patients who lived in urban areas. Working patients were 2.5 times more likely to have suicidal thoughts prior to surgery. Severely traumatic events increased the chances of suicidal ideation (OR 7.72; 95% CI 1.63–36.6; p = 0.01). The symptoms of PTSD showed a threefold increase in the likelihood of suicidal ideation (OR 2.89; 95% CI 0.98–8.55; p = 0.05). Conclusions: Living in the countryside, having a history of traumatic experience, having a paid job and having symptoms of post-traumatic stress disorder influence suicidal ideation in breast cancer patients. Particular attention should be drawn to individuals with multiple risk factors.
