Community Pharmacists’ Knowledge, Willingness, and Readiness to Prescribe Oral Contraceptives in Saudi Arabia

Background: The role of community pharmacists (CPs) in various healthcare settings is well documented in the literature including providing safe and easy access to medications. Oral contraceptives (OCPs) are the most frequently used method of terminating unwanted pregnancies worldwide. Objective: This study aims to evaluate the Community pharmacist’s knowledge, willingness, and readiness to prescribe OCPs in Saudi communities in Saudi Arabia. Methods: This is across sectional, self-administered questionnaire-based study conducted between May and November 2021 in the central region of Saudi Arabia. The results were presented as frequencies and percentages. Chi-square tests were used to sort significant association between groups. Results: Out of 368 CPs who got the questionnaire, 347 completed (94.3%). Most of CPs were be-tween the ages of 25 and 35, with 76.9% working in chain pharmacies. Of the surveyed CPs, 45.5% had >24 months of experience in community pharmacies. 41.8% of them prescribed more than six prescriptions for OCPs/week. The patients’ safety (77.2%), physician’s resistance (54.5%), and CPs objection based on religious purposes (36.9%) and lack of time (29.7%) were the most commonly cited barriers among CPs. CPs who worked in chain pharmacies were significantly too busy (p = 0.038) to prescribe OCPs. Also, community pharmacists with experience of more than two years significantly agreed not to prescribe OCPs due to religious convictions (p = 0.009). Conclusion: The current study revealed that most of the CPs were knowledgeable about OCPs. Additionally, most of them were likely to prescribe oral contraceptives. We further suggest overcoming the barriers associated with contraceptives among CPs and providing sufficient training to improve the oral contraceptive prescriptions in CPs is needed.

Unforeseen uses of oral contraceptive pills: Exploratory study in Jordanian community pharmacies

Background The use of oral contraceptive pills (OCPs) as a birth control method is very common worldwide. OCPs have many other labeled non-contraceptive indications, and as a result there is an associated risk of improper use, as with any other medications. This study was designed to assess the unforeseen improper uses of OCPs observed by community pharmacists in Jordan. Method A cross-sectional study design was conducted using a self-administered survey. A convenience sample (n = 380) of Jordanian community pharmacists, were recruited through social media resources. The survey included multiple-choice and open-ended questions. Descriptive statistics and correlation analyses were completed using SPSS. Results More than half of the recruited pharmacists (55.3%) were female, and the mean age of the participants was 32.58 ± 9.94. The majority of the pharmacists (85%) had good knowledge about the non-contraceptive indications of OCPs. About 53% of them confirmed their exposure to cases of the improper use of OCPs. About 67.5% of the pharmacists who confirmed exposure to such cases, reported the topical use of OCPs for the enhancement of hair growth. Around 15% of those pharmacists stated that OCPs were used to give negative results for addictive drug screening tests. In the event that the pharmacists suspected improper use, more than 90% suggested they would refrain from dispensing the pills. Conclusion This study has spotlighted many unforeseen uses of OCPs in Jordan and highlighted the need for restricted national regulations on the monitoring of OCP prescription/selling patterns in Jordan by policymakers. Moreover, there is a need for the establishment of national educational programs for the Jordanian community regarding the safe proper use of OCPs.

Kontrasepsi oral (pil KB) merupakan salah satu metode kontrasepsi dengan tingkat efikasi yang baik jika diminum dengan baik dan rutin. Tingkat pengetahuan yang baik terhadap kontrasepsi oral akan memberikan dampak positif pada akseptor KB. Tujuan penelitian ini ialah untuk mengetahui tingkat pengetahuan akseptor KB terhadap kontrasepsi oral dan faktor-faktor yang mempengaruhi tingkat pengetahuan. Penelitian ini merupakan penelitian analitik prospektif observasional dengan desain penelitian cross-sectional. Penelitian ini menggunakan kuesioner dengan skala guttman yang telah tervalidasi untuk mengukur tingkat pengetahuan akseptor KB oral. Sampel pada penelitian ini adalah akseptor yang menggunakan pil KB yang memenuhi kriteria inklusi. Teknik pengambilan sampel menggunakan non-probability sampling dengan jenis consecutive sampling. Data dikumpulkan pada bulan Desember 2019 hingga Maret 2020 di tujuh apotek daerah Surabaya Timur. Didapatkan 98 akseptor KB oral yang memenuhi kriteria inklusi. Hasil penelitian menunjukkan tingkat pengetahuan akseptor KB oral terhadap kontrasepsi oral berada dalam kategori sangat baik sebanyak 14.3%, baik 52.0%, cukup 8.2% dan sangat rendah 25.5% dengan skor rata-rata pengetahuan sebesar 6.8 yang tergolong dalam kategori pengetahuan baik (6.6-8.0). Dari uji korelasi Spearman, hanya variabel tingkat pendidikan yang memiliki hubungan yang signifikan dengan skor pengetahuan (r=0.484; p value = 0.00). Kesimpulan pada penelitian ini ialah tingkat pengetahuan akseptor KB terhadap kontrasepsi oral termasuk dalam kategori baik serta faktor pendidikan memiliki korelasi secara signifikan terhadap tingkat pengetahuan akseptor KB oral.
