ÖZ Nazogastrik tüpler gastrointestinal dekompresyon, enteral beslenme, ilaç uygulaması, nazobiliyer drenaj ve üst gastrointestinal kanama gibi çeşitli endikasyonlar için kullanılır. Yoğun bakım ünitelerinde beslenmenin yanında bazı ilaçların verilmesi için de sıklıkla hemşireler tarafından nazogastrik tüpler kullanılmaktadır. Gastrik yolun kolay erişim, erken erişim ve hemşire tarafından yerleştirilebilme gibi avantajlarının yanında, pozisyon doğrulama zorluğu, nazofaringeal travma, tüpün yer değiştirmesi ve aspirasyon riski gibi dezavantajları da bulunmaktadır. Olgumuzda amacına uygun olmayan nazogastrik tüp kullanımı sonucu tüpün ucunda oluşan düğümün neden olduğu mekanik obstrüksiyon durumu sunulmaktadır. Nazogastrik tüplerin doğru ölçümler ile uygun seviyede takılması, her kullanım öncesi yerinin doğrulanması, tıkanıklık olup olmadığının değerlendirilmesi ve seviyesinin düzenli olarak kontrol edilmesi ile birçok komplikasyon önlenebilmektedir. Hastanın tedavi ve beslenmesinin aksamaması için komplikasyonların önlenmesi büyük önem taşımaktadır. Bu bağlamda kesintisiz 24 saat bakım hizmeti sunan yoğun bakım hemşirelerine hayati sorumluluklar düşmektedir.ABSTRACT Nasogastric tubes are used for various indications such as gastrointestinal decompression, enteral nutrition, drug administration, naso-biliary drainage and upper gastrointestinal bleeding. In addition to nutrition, nasogastric tubes are frequently used by nurses in order to provide some medicines in intensive care units. Besides the advantages of easy and early access to the gastric tract, and placement by the nurse, there are disadvantages such as position verification difficulty, nasopharyngeal trauma, tube displacement and aspiration risk. In our case, we present a case of mechanical obstruction caused by the knot formed at the end of the tube as a result of the use of nasogastric tube that was not suitable for the purpose. Many complications can be prevented by correct placement of nasogastric tubes, verification of the location before each use, evaluation of obstruction and regular control of the level. Prevention of complications is of great importance in order not to interrupt the treatment and nutrition. In this context, intensive care nurses providing uninterrupted 24-hour care services have vital responsibilities.

“…The most reliable method to confirm correct tube position is by the use of imaging, commonly plain film radiography, whereby correct position is demonstrated by confirmation of the tube clearly passing below the diaphragm, not deviating into either of the main bronchi, and with its tip evident within the stomach 6. If excessive tube length within the stomach is evident on imaging it should be withdrawn by the desired amount and tube position should be re-checked with further imaging.…”

Nasogastric tube placement is a common procedure performed in surgical and medical specialities. The occurrence of knot formation is perhaps one of the least well-recognised complications associated with its usage. We present a case of nasogastric tube knotting to remind colleagues of this rare but important complication. A 75-year-old woman with adhesional bowel obstruction was admitted under the general surgery team. A wide bore nasogastric tube was inserted for drainage and decompression. Although placement of the tube was deemed to be successful, there was no drainage of gastric content evident. On removal of the 'non-functioning' tube a simple knot was seen at its proximal end. A further nasogastric tube was placed and the patient's symptoms resolved with conservative management. Nasogastric tube knotting is a rare and often overlooked complication with the potential to cause significant trauma on tube removal if unrecognised.

<p>Objetivo: conhecer os eventos adversos relacionados à inserção, manutenção e remoção de sonda enteral descritos na literatura. Método: trata-se de uma revisão integrativa de literatura. A estratégia PICO foi utilizada para formulação da questão norteadora. Foram selecionados artigos das bases PubMed, Embase e Scopus, publicados entre junho de 2009 e junho de 2019, nas línguas portuguesa, espanhola e inglesa. Resultados: foram incluídos 45 estudos, categorizados em eventos adversos relacionados à inserção da sonda enteral (n=22) que promoveram danos como epistaxe por lesão até a inserção de sonda no átrio direito; eventos relativos à manutenção da sonda e/ou administração de dieta (n=14), como a infusão de dieta em sítio pulmonar; e aqueles ocorridos na remoção (n=9), como emaranhado na ponta distal. Conclusão: diferentes eventos adversos descritos na literatura podem ocorrer na inserção, manutenção, administração de dieta e remoção da sonda enteral.</p><p>Descritores: Intubação Gastrointestinal.
